Witness:
LOVELY L. DE LA TORRE, of legal age, Filipino, and resident of Ungka
II, Pavia, Iloilo.

Formal Offer: Your Honor, we are offering the testimony of Plaintiff Lovely
L. De La Torre who will be testifying as to the material allegations in her
complaint against Jury Jason B. Junsan. Plaintiff, in the course of her
testimony, will identify documents pertinent to this case and will also testify
on other allied matters.

I, LOVELY L. DE LA TORRE, of legal age, Filipino, single, and a


resident of Ungka II, Pavia, Iloilo, after having been duly sworn to in
accordance with law, hereby states that:

I am answering the questions propounded by my counsel, Atty. Abundio


A. Bacatan of Bacatan Law Office, fully conscious that I am under oath and
that I may face criminal liability for false testimony or perjury;
This Judicial Affidavit is written in English, a language known and
understood by me, and the examination and execution thereof were done at
Bacatan Law Offie, Don Bosco Boys’ Home, Dumangas, Iloilo; and

This Judicial Affidavit shall serve as my direct testimony in this case.

1. Q: Please state your name and other personal circumstances.


A: I am LOVELY L. DE LA TORRE, of legal age, Filipino, single, and
a resident of Ungka II, Pavia, Iloilo.

2. Q: Are you the same LOVELY L. DE LA TORRE, who is the plaintiff


in civil case no. 456 for collection of sum of money filed against JURY
JASON B. JUNSAN?
A: Yes, Sir.

3. Q: Why are you here in court today, Miss Witness?


A: To give my testimony in connection with the instant case I filed
against JURY JASON B. JUNSAN Sir.

4. Q: How are you related to JURY JASON B. JUNSAN?


A: He is a good friend of mine.

5. Q: Do you remember having met JURY JASON JUNSAN sometime


in October 2016?
A: Yes, Sir.

6. Q: Why did you meet with him?


A: He wanted to borrow money from me so we met in a coffee shop
and I lent him Five Hundred Thousand Pesos (P500,000.00).

7. Q: Why did you agree to lend him that amount of money?


A: As I said, he is a good friend of mine. Our families have a good
relationship. And his loan is with interest.

8. Q: What are the terms and conditions of his loan from you?
A: Well, we agreed that his loan of Five Hundred Thousand Pesos
(P500,000.00) shall be payable in 12 months with an interest of twenty
percent (20%) in a year.

9. Q: When was he supposed to start paying?


A: He was supposed to start his monthly installment one month after
he borrowed the money, that is on November 17, 2016.

10.Q: And the loan is supposed to be due on?


A: On October 17, 2017.

11.Q: Now, how much was he able to pay you, if any?


A: He was only able to pay a total sum of Two Hundred Thousand
Pesos (P200,000.00). He first paid One Hundred Twenty Thousand
Pesos (P120,000.00), then Eighty Thousand Pesos (P80,000.00).

12.Q: When did he make these payments?


A: The first one was on February 17, 2017 and the second one was on
April 17, 2017.

13.Q: After April 17, 2017, did he not make any payment?
A: No Sir. I have not received any payment after that.

14.Q: Considering what you have said Miss Witness, what do you want
the Honorable Court to do with the case you filed?
A: I hope that this case will be decided with fairness and that the Court
will order the defendant to pay me the balance of his loan plus the
interest rate and surcharges, and pay me the litigation expenses and
damages.

15.Q: Do you affirm and confirm the truthfulness and veracity of all the
statements you stated in this judicial affidavit?
A: Yes Sir.
