Target Audience:
o Patients admitted or diagnosed with Congestive Heart Failure (CHF)
Audience Characteristics:
o Varying ages; primarily >60 years old
o Different health backgrounds, physical capabilities and life experiences
o May be retired or still working full/part-time
Educational level:
Varying educational levels from high school diploma to PhD
Some have never discussed nutrition-related topics in depth before
Concerns: Motivation and willingness of patients to learn ways to change their lifestyles
and make changes to better manage their CHF.
Advance arrangements/contacts:
o Intern has Low Sodium Diet handout available to review with patients
Behavioral Objectives:
o For patients to be able to identify 3 high sodium foods that are commonly a part
of their diet; be able to understand explain label reading in regards to sodium
content
Visuals:
o Low Sodium Diet handout
o Additional educations materials made available to patients can be found in
Module 5: Education and Counseling
Pre- and post-test (Use creative approaches; plan method of administering the pre-
and post-test that is appropriate to the audience.)
o Pre-test: Ask patients to identify 3 or more high sodium foods they often eat or
keep at home. Also, ask them if they have ever followed a low sodium diet or any
other specific diet in the past/with or without success.
o Post-test: After reviewing the foods both high and low in sodium, label reading
and meal timing from the Low Sodium Diet handout, review this information and
ask the patient to list 3 or more foods high in sodium
Schedule: How long each item will take?
o Introduction and Discussion – 2 minutes
o Brief review of Low Sodium Diet and CHF – 3 minutes
o Explain sodium content of foods, meal timing and label reading – 10 minutes
o Clarify any underlying questions, discussion and review food choices – 5 minutes
Body of presentation
o Review handout information including both foods to limit and include in daily
diet; explain label reading
Conclusion /post-test
o Review with patient and family members why a low sodium diet is the best option
and should be a primary concern compared to other diet restrictions; low sodium
food options to incorporate in diet and consider while cooking in addition to
brainstorming ideas on what they can be doing differently moving forward to
maintain good health and manage their Congestive Heart Failure
