This pertains to your Residential Free Patent Application over Lot No. 374, Cad
1191 D located at Brgy. Poblacion,Dimasalang, Masbate, with an area of 198 sq. m. filed
before our office last February 2012.

Please be informed that per evaluation thereof, it appears that the following
requirements are lacking, to wit:

1. Zoning Certification – CPDO/ MPDO

In this connection, please be advised to submit the aforecited documents on or

before July,25,2017 in order for us to expedite the processing and issuance of
title/tenurial instrument in your name.

Your preferential attention on this matter is enjoined as the failure to submit the
requirements may be among the grounds for the cancellation of your application.
