No. SSB-PRIMOS SUHATI SDN BED Revision No 7 Bfective Date 26/09/2014 METHOD OF STATEMENT Page gait ee __ 1.0 CCTV Survey 1.1 Method Statement The following passages are extracted from our STANDING OPERATIONAL PROCEDURES as a guide to our method of operation. 1.2 Scope To inspect sewerage system, storm water system and associated manholes/entry points by Closed Circuit Television (CCTV). A CCTV inspection Is designed to ascertain the condition of the foul and storm water drains and in particular, to provide data assessing: +The structural integrity of the pipeline system. * The service condition of the pipeline system. * The possible cause and effect of any extraneous water infiltration/exfiltration to/from the pipeline system. ‘* The location and extent of expedient connections to the pipe line system, The location and structural condition of associated manholes. 1.3. The Team Leader will ensure his satisfaction of the issued documents by his signature in the site works register. Lighting, Signing and Guarding Manhole Covers arc not opened until Lighting, Signing and Guarding of Roadworks or Footway works in compliance with the Road Traffic (Traffic Control)Regulations Code of Practice and the Department of Transport Chapter 8 (Traffic Safety Measures for Roadworks). The Company will comply with all statutory regulations and enactment and particular specific instructions from our clients, Expanded Operational Procedures for CCTV Operation On arrival at site, the crew leader will determine the most logical approach for the inspection, In normal circumstances, the inspection will be Undertaken in a down stream direction, this is not however, always the case, Jn some circumstances, it may not be possible to mobilize the equipment at the desired start point, therefore a down stream start point will be chosen and the inspection will be carried out up stream. Having decided on the start point the crew leader will deploy the vehicle in close proximity to the start manhole. The exit and entry manhole covers will be removed to allow for venting of the pipeline. The two areas will then be secured using the current code of practice for coning & signing of ‘open manholes, Ref. Whilst this is under way, the crew leader will input the inspection header information onto the monitor screen and loading the videotape into the video recorder. The following information will be displayed: Page 1. Customer name Location of inspection (Street Name etc,) Date of inspection Page 2. No mixed information ‘The crew will, by now, have connected the camera unit to the main cable and mounted the camera on the skid / tractor. The crew leader will test the camera, the camera lights, the camera focus and the tractor units to ensure all are in working order. If the camera is to be winched through the pipeline, the crew will pass the towing line through the pipe from the U/S, D/S manhole and connected this to the camera skid, The pipe diameter will have been measured and the camera height a Ref. When all tests have been completed, the camera is inserted into the pipe Tun and the slack is taken out of the main cable. The crew leader will then adjust the metercounter to zero, The meter counter is activated by the camera cable turning a counter wheel on the cable drum. This transmits a signal to the control unit and the counter is displayed on the monitor screen. As the camera moves along the pipe, the counter increases in 10- m increment to an accuracy of within 2%. ‘The crew leader adjusts the light and focus to show the best picture on Monitor. He then informs the crew that the inspection is about to commence and activates the video recorder and the tractor unit, The camera then travels along the pipe at an average speed of 10-cm per second (or other standard speed), being the optimum speed that will allow the crew leader to identify all faults and record them. The crew leader will note all defects but will take photographs of major faults either by off screen photography or by digital "Videoprint" Photography, numbering each shot consecutively within each survey. At the end of each survey, the crew leader will close down the survey by entering page 3 information stating the manhole number and the total chainage measured on the counter. This procedure is used for each Survey, @ survey being from MH to MH. At the commencement of each survey the meter counter is reset to zero. On completion of survey, the videotapes are returned to the office where the information is compiled into report format either through manual or computerized operations, dependent on customer requirements, The format is that laid down in the Manual of Sewer Condition Classification ef. The computer version displays the same information but in a slightly different layout, All photographs are mounted and inserted behind the relevant page in the report, thus providing the customer an instant view of the fault, without the need to view the videotape. The negatives of screen photography will be contained in an envelope at the back of the report, For "Videoprint" photographs, the videotape Is the negative. ‘The CCTV inspection is used for pipe diameters up to and including 1200mm. For pipelines over this size, In Conduit Photography (ICP) is used, 2.0 In Conduit Photography/Man-Entry Inspection In conduit photography (ICP) is utilized in the large diameter pipes of 1200mm or greater. The reason for ICP is that with the large diameter pipes, the distance from the camera lens to the object is too great to provide a clear picture on the CCTV monitor screen and hence, faults may not be easily identified, By using ICP the ICP photographer can take close up shots of defects, both point and continuous. The procedure is as follows: ‘The ICP team comprises 2 men, One man being the photographer, the other being the recorder. At the start of the traverse, the team leader, using surveyors spray paint, will mark a vertical line on the pipe wall at the 0900 clock reference and number this zero. At each 5m point, he will make a similar mark and increase the numbering by 5m at each point. The numbers should be approximately 12" in height. The distance is measured using either a meter wheel or a surveyor tape, The photographer will take a shot at each 5m mark starting at zero, ensuring that the camera is in the center of the pipe axis and that the number is just visible Ref. He will take individual shots of all points and continuous defects and the recorder will jog each shot stating what the shot relates to. At the end of the traverse, the recorder transcribes the information onto the Prescribed forms to ensure that no information is missed or lost. The films are developed as early as possible after the inspection and then mounted and details added. The ICP report is in the same format as that for CCTV. In certain circumstances, it is possible to use the CCTV camera in place of the still camera. The circumstances being where distance accuracy is of minimal importance and purely a condition inspection is required. CCTV Inspection of Water Mains ‘As water mains are pressurized systems, entry into the mains must be via a valve connection or via a blanking plate. The entry must be supervised or undertaken by an engineer from the relevant Water Supplies Department. This will require forward planning as with the removal of a valve/blanking plate, and the following points must be considered. ‘+The effect on users having their water supply cut for the duration of the inspection; + The drain down of the system; ‘+ Probable damage to seals during the removal of valves/blanking plates, and the requirement to have new items of the correct type and size available before removal of valves etc; * The requirement for a Water Supplies Department engineer to either supervise or undertake the removal and replacement of the valves/blanking plates, to ensure that there is no cause for call back at a later date, should a fault develop in the system inspected. All pipe depths should be taken from this ‘T’ point vertically and not ‘on an angle.The way to do this is to measure the shaft depth D and then measure the depths from where the pipe enter the chamber to the underside of the slab and add the two together, i.e, D + F, D +EorD+G Re. IMPORTANT 4.2.1 Crew leader must be satisfied that his crew members are taking Measurements accurately. If he doubts a measurement given for Gepth, chamber size, pipe size etc as well as construction details and materials. He must get the crew member to re-check the measurement or details. 4.2.2 Crew leaders should not go into the manholes to take measurements unless he wishes to check for special items or details, 4.2.3 Crew leader should be the only member of the team, which fill out the manhole record forms to ensure the consistency. 4.2.4 When a slope is completed the crew leader should check his work to ensure that all details are correct and connectivity is shown on a neat sketch. 4.2.5 Assume the Cls are the same, the above diagram will help to prove the connectivity. 4.2.6 If the number of incoming pipes is more than 23, use W, W1, W2, etc. 4.2.7 Standard sizes for concrete pipes are as follow: 100, 150, 225, 300, 375, 450, 525, 600, 750, 900; 1050, 1200, 1350, 1500 etc, a) When measured size is less than 10mm from the standard size, use REN SEM SUHATI SDN BHD Revision No. o Effective Date, 26/09/2014 METHOD OF STATEMENT Page ‘12 of 26 a standard size. b) When measured size is more than 10mm from the standard size use actual size. Example: Measured Sized 90mm 100mm 595mm 600mm 80mm 80mm 100mm. 1000mm 550mm 550mm Method of Measuring of Standard Pipe Size: 5.0 Working in Confined Spaces: Working in Confined Spaces can be hazardous so strict observance of all safety procedures is paramount. Team Leaders shall make a risk assessment and fully brief, all team members to ensure, they all know the tasks they are to perform, in accordance with Working in Confined Spaces, Health and Safety at Work Act 1974, 5.1 Personnel REN SSEPRIMDS SUHATI SDN BHD Rae : Effective Date BOODLE METHOD OF STATEMENT — TB of De —————— Persons who may be required to enter confined spaces should be given a medical examination before commencement of employment and, if possible, at reasonable intervals thereafter. If any of the following disabilities are found, the person should, where Possible, be given other work and not enter confined spaces. a) Fits, blackouts or fainting attacks, b) Heart disease or disorder. ©) High blood pressure, d) Asthma, bronchitis or shortness of breath on exertion. ©) Deafness. f) __ Menieres disease or disease involving giddiness/loss of balance. 9) _Claustrophobia or other nervous or mental disorders. 6.0 Entry into Confined Spaces a) Definition A confined space is any area where the free interchange of uncontaminated air is prevented and where it is reasonable to expect the presence or prospect of @ hazardous atmosphere. This includes open tanks and channels that may have gases heavier than air trapped in them creating a hazard, A secondary feature of confined spaces is that they usually have a restricted access; this may make entry into and exit from them particularly difficult. b) Dangers The purpose of this section is to draw attention to the more common hazards likely to occur in sewer and manholes. It is not a complete of dangers. If there are any doubts to the safety of a confined space, DO NOT ENTER. Do not be forced to undertake any tasks considered as [ Ref. The Company will always support your decision for not to proceed. |, Fall hazard - due to the fact that sewers are old, sometimes badly constructed and often subject to damage from sewage, it is unwise to assume that a sewer checked yesterday is safe today. Manhole ladders and step irons should be regarded as they can fail unpredictably under load. Each rung and step iron should be tested prior to weight being placed upon it. Safety harnesses, lifelines and man winches (where required) must be used. Ensure the top man has a firm grip on the rope as you descend, do not allow slack to build up in lines or wires. suspicion Atmosphere Hazard - the atmosphere in a sewer can become Potentially dangerous due to the presence of components, which are toxic or inflammable, or through a deficiency or excess of ‘oxygen. These conditions may arise from the decomposition of deposited solids or from a variety of less predictable occurrences such as leakage from gas mains, the discharge of trade effluent and spillage of petrol or chemicals some of which may be individually harmless but dangerous when mixed. Even after a manhole has been tested and found to be safe the atmosphere can gradually or suddenly change for the worse. An unusual smell, dizziness, nausea, throat or eye irritation can be an indication that something is wrong. Should this happen, or should the gas monitor alarm, the sewer should be evacuated and further investigation undertaken. iil, Flood Hazard - a sudden rush of water may be caused by: Ref. No, [__SSB-PREMOS SUHATI SDN BHD Revision NG: 5 Effective Date 2670972014 METHOD OF STATEMENT ra Boras ——— ——_ Arain storm, possibly remote from the work site, The rapid release of a large volume of water from swimming pool, reservoir etc.The failure or unauthorized operation of valves, penstocks, stank, etc. Therefore, always check local weather conditions and warn colleagues of rainfall.Ensure all valve gear is locked off with a sign starting that men are working in the sewer. ‘Temporary or old penstocks and stank should be inspected prior to entry.Also listen for the unexpected sound of rushing water or a surge of wind which may indicate a sudden inflow of water. It is important, especially during sewer traversing, to be aware that flows may rise slowly. A depth check should therefore be made at regular intervals. lv. Chemical hazard - clients should warn contractors of licensed discharges into sewers and watercourses, which may pose hazard to health. However, it should not be assumed that where no warning has been issued no hazard exists. Indeed, all flows should be treated as suspect as chemicals, which are innocuous on their ‘own, may become noxious when combined together in a sewer. Any unusual substances should be investigated and the sewer should not be entered until proven safe. Some symptoms, caused by hazardous chemicals, are irritated eyes, nose, skin and lungs. Difficulty may be experienced in breathing while you may also feel dizzy or sick. Good hygiene procedures will prevent many illnesses and Infections, these are dealt within the in the Safe System of Work Section D. 7.0 Quality Checks ‘The following is a list of checks to be carried out before any reports are submitted for validation, 7.1 CCTV Reports * Check that the cover accurately displays the contents of the file * Check that the ‘Examiner’ summary contains all the surveys within the section and agrees with the cover. * Check that the drainage schedule displays all the surveys within the section * Check that all depths and levels are entered on the drainage SUHATI SDN BHD oe SSEPRIMTS METHOD OF STATEMENT Pa ae —— schedule * Check that the defects summary agrees with the relevant AMPs survey sheet ‘© Check that the correct defect code has been used * Check that all header information on drainage schedule and defects summary are consistent + Check that the correct 8 fig STC Grid Reference is used to identify manholes * Check that all pipe sizes and material agree on all forms and drawings * Check that all spelling is correct * Check that the floppy discs contain all information within the file and ensure that these are secured in the file prior to submission + Check that there is a photograph for each defect reported on the AMPs report sheet * Check that all photographs are clear and give a good representation of the fault + Check that the correct information has been endorsed on the photograph sheet ‘+ Check that all negatives are present and presented with the master copy of the report * Check that any amendment to the AMPs report that results in conflicting information on the video tape, is identified on the bottom of the AMPs report * Check that photograph negatives are all present and correctly identified and incorporated in the master copy of the report. 7.2 Drawing (CCTV Inspection & Manhole Survey) * Check that all manholes noted on the base plan have been incorporated in the report.
