CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

THIS CONTRACT, entered into by and between:

_________________________________________________________, of legal age, Filipino citizen,


_________________________ and with residence/office address at __________________________________
_________________________________________________________, hereinafter referred to as the LESSOR;

- and -

JESUS IS LORD CHURCH, a religious corporation duly organized and existing under the laws of the Republic of the
Philippines with office address at 3rd Floor, St. Francis Square, Julia Vargas Avenue cor. Bank Drive Mandaluyong City, and
duly represented by CARMENCITA L. TOBIAS, hereinafter referred to as the LESSEE;

WITNESSETH, that:

WHEREAS, the LESSOR is the attorney-in-fact of ______________________________________________ for that real property
located at ___________________________________________________________________________________
___________________________________________________________ covered by Original/Transfer Certificate of Title/RPTD No./
__________________________________________________________ duly authorized to execute this Contract. A copy of the Special
Power of Attorney in favor of LESSOR is attached and made an integral part hereof as Annex “A” ;

WHEREAS, the LESSEE desires to lease the above said property specifically the

_______________________________ (________) square meters, more or less, hereinafter referred to as the Leased Premises;

WHEREAS, the LESSOR has agreed to lease the Leased Premises to the LESSEE subject to the terms and conditions set forth
hereunder;

NOW THEREFORE, for and in consideration of the payment of rent and of the terms and conditions hereunder contained, the
parties have agreed as follows:

1.The lease shall be for a period of ____________________ (_____) months beginning on _________________________________
20____ until _____________________________ 20_____ renewable upon mutual agreement of the parties;

acknowledged by the LESSOR. The security deposit shall be refundable to the LESSEE at the end of this lease, upon clearance of payment
for utilities incurred up to the end of lease contract;

4.The Leased Premises have been received by the LESSEE in good and tenantable condition;

5.All real property taxes and other assessments on the Leased Premises shall be for the account of the LESSOR;

6.All ordinary repairs within the Leased Premises that arise in the daily use of the facilities therein shall be for the account and expense of
the LESSEE, without right of reimbursement. Major repairs, however, are for LESSOR’s account. LESSEE should, however, give prior notice
to LESSOR for whatever improvements to be made for approval by the latter: Provided, that such approval shall not be unreasonably
withheld: Provided further that the LESSEE may remove at the end of the lease any improvements it has made on the Leased Premises
which are detachable in nature and could be removed without causing major damage to the Leased Premises;

7.Save ordinary wear and tear and other events not attributable to the LESSEE, the LESSEE shall deliver the Leased Premises to the
LESSOR at the end of this lease or termination thereof in as good condition in all respects as when first occupied;

8.The LESSEE shall use the Leased Premises exclusively for church and office purposes, with right of the LESSEE to have caretaker/s to
reside on the premises for proper maintenance;

9.The LESSEE is expressly prohibited to sublease the Leased Premises to anyone without the written consent of the LESSOR;

10.All charges for water, light, gas and telephone used within the Leased Premises for the duration of this contract shall be for the account of
the LESSEE;

11.The LESSEE shall be responsible for the observance of sanitary and electrical regulations required or imposed by the city or government
authorities regarding the use of the Leased Premises;

12.The LESSEE shall have the right to preterminate the lease provided that proper notice in writing shall be made to the LESSOR at least
thirty (30) days prior to the intended date of pretermination;

13.In case of sale, transfer or other disposition by the LESSOR of the Leased Premises or property, LESSOR hereby gives the LESSEE the
right of first refusal to acquire the Leased Premises and/or of the entire property covered by the aforecited certificate of title. Notice of such
intent to sell, transfer or dispose of the property/Leased Premises shall be in writing and sent to the LESSEE at least sixty (60) days prior to
such sale or transfer;

14.In case of mortgage or other encumbrance without transfer of ownership, LESSOR undertakes, agrees and warrants that the mortgagee,
transferee or assignee shall respect the terms and conditions of this Contract of Lease and that written notice shall be made to the LESSEE
at least sixty (60) days prior to such mortgage, encumbrance or other disposition, short of transfer of ownership, of the Leased Premises;

15.This Contract shall bind the parties herein, their heirs, assignees and successor/s-in-interest;

16. Any action or suit in connection with this Contract of Lease shall be instituted in the proper courts of Pasig City to the exclusion of other
venue.
