AFFIDAVIT
I, ANGELINA G. GUTIZA, single, Filipino, of legal age and resident Blk 24 Lot
31 Ph 1, Cityhomes Resortville Langkaan 2, City of Dasmariñas, Cavite, after being duly
sworn to in accordance with law, depose and state: That - -

1. I am the daughter of SPS. Nelson G. Gutiza and Josefina Godoyo,

who was born on January 24, 1966 at Bulan Sorsogon;

2. I have never contracted any marriage in the Philippines or elsewhere and

I remain SINGLE to this very day and hour;

3. There is no legal impediment for me to contract marriage under the

Philippine Laws;

4. Unfortunately, I used my common law husband surname “PABLO”

instead of my surname “GUTIZA” in all my government identification
cards;

5. Through this affidavit I hereby declare under oath that my true and
correct name is “ANGELINA GODOYO GUTIZA”, hence the names
“ANGELINA GODOYO GUTIZA” and “ANGELINA GUTIZA PABLO” is
one and the same person;

6. I am executing this Affidavit for the purpose of complying with

the requirements for my government record;

7. I declare that the statements made in this affidavit are true and correct
and I am willing to bear all the legal responsibilities arising there from.
