TRANSFER OF OWNERSHIP OF ELECTRICITY SERVICE CONNECTION

AGREEMENT

Residing In D.No. ___________________ Street, Tirupati here by agree to transfer
the SCNO ______________ distn. to Sri/Smt _____________________________
together with the S.D held against the service with the S.P.D.G of A.P Ltd., Since I
have disposed off transferred all my properties belonging to the above said SC NO
SCNO and handed over possession of the same transferee.

the properties belonging to the above said service connection I hereby agree to
abide by the terms and conditions of supply laid down by the S.P.D.C of A.P Ltd.., as
agreed to by the transfer and I am prepared to execute a fresh agreement if
required as per Rules and Regulations.

Service Connection No ________________ of APSPDCL, ERO/Tirupati standing the
name of Sri /Smt ________________________ S/o / W/o ___________________
to my name .1 also produce a copy of Regd. Sale deed from the appropriate
authority .The service Connection premise is bearing the H.No. ________________
Located at ___________________________ with the Electricity Section Office.

However I am prepare to face all the legal consequences that may if ariseon having
transferring the SC.NO. ____________ from Sri _________________________ to
my name.
