INDIVIDUAL INTAKE FORM

Practicum in Professional School Counseling

Name: Allyson Harter Grade: 7 Date: November 10, 2018

Nature of Client: Allyson is a very sympathetic and shy seventh grade female who struggles regulating her
emotions. We met one day when I was monitoring students at lunch and her friends brought her to me, she
was in tears. She was hesitant to speak with me at first, but her friends convinced her that I “am really
nice.” At first Allyson talked about not fitting in, not feeling herself, and not adjusting well to a new school
building. These are typical feelings of a seventh grade student, but she could hardly speak about it without
crying. I talked with Allyson that day and asked her if she would like to meet with me more regularly. She
then asked me if we could meet every Friday. I said absolutely! We have been meeting every week since
and we talk about general things, like fitting in, technology, being a teenager, and getting along with
siblings.

Summary of Interviews: The best part of my various sessions with Allyson is that if I forget to get her out of
class on Friday, she comes on her own at the very end of the day and I stop what I am doing to meet with
her. When students are seeking out a relationship with a counselor, you nurture that relationship, not only
for that individual student, but to build rapport with the entire student body. Students talk to one another
about what adults are like and the adult’s attitude about students and time spent with them. I have gained a
lot of confidence in individual sessions and how to build on previous sessions to instill growth in the
individual. The hope is that the individual gains skills in emotional regulation, self-awareness, social-
awareness, relationship skills, and decision making.

Interview Outcome: Allyson feels much more comfortable in the school building having a supportive adult
contact she can speak with in the hallway and meet with once a week. Allyson is becoming more confident
in who she is and understanding that differences are a good thing and make the world a better place. She is
also beginning to understand that it is perfectly normal for a teenager to have the feelings she does and that
showing emotion with these feelings is better than trying to hold emotions in. Allyson has grown to enjoy
our relationship. She even chose me as her “role model” for spirit week. I count this as my first success as a
school counselor.

Additional Comments: I wish I had enough time to meet with 20+ students on an individual basis each
week. I am hoping that I will feel more available next year to meet with more students because student-
adult relationships are very valuable in creating a positive relationship with school.
