ADULT MODEL RELEASE AGREEMENT

The Parties to this legally binding agreement, _______________________________________

(hereinafter “Photographer. We, Us, Ours, etc.”) ; and _______________________________
(hereinafter “Model”, You, Your, Yours, etc.”) collectively hereinafter “the Parties”, enter into the
following release agreement, which shall be a forever legally binding contract at law. The terms
of contract are as follows:

1. CONTROLLING LAW- The parties agree that the controlling and governing law of this agreement
shall be the laws of the state of Oklahoma, and the federal laws of the United States of America, as both
may ordinarily apply. This shall be to the express exclusion of the governance of the law of any other
State or Territory.

2. ENTIRETY OF THE CONTRACT- The parties agree that this contract represents the entirety of
the agreement in contract, and that the agreement may not be modified, except in writing, and signed
by both parties. This is to the express exclusion of any and all oral contracts, or similar.

3. SERVERABILITY- The parties agree that if any term of this contract shall be determined by a
court of law to be non-enforceable for any reason, the remainder shall remain in full force and effect at
all times.

2. AGE OF MODEL - The model represents and warrants to the Photographer that the model is at
least eighteen (18) years of age or older, and agrees to provide any and all documentation (such as a
state issued photo-identification card) which is deemed necessary by the photographer to comply with
U.S. federal laws at 18 U.S.C. 2257 and/or 18 U.S.C. 2257A. The model understands that making false
statements about age whatsoever, and/or uttering forged identification documents may constitute a
criminal felony under the law, and that the photographer will prosecute to the full extent of the law, in
efforts to enforce the anti-child pornography provisions of the federal CCPA. The model states that

names for similar types of photography in the past: ______________________________. The model
further represents and warrants that the model is legally competent to enter into this contract.

3. SCOPE OF PHOTOGRAPHY - The parties agree that the model is being photographed by the
photographer on the date(s) Of:_________________. The model understands and agrees that the type
of photography will be of a homosexual pornographic nature. The model understands and agrees

4. SCOPE OF RELEASE- Subject to the other provisions of this contract, the model agrees that the
model is granting the photographer the unrestricted, irrevocable, and worldwide permission to use and
publish the photographs and/or motion pictures produced as a result of this agreement, in whole, or in
part, with or without alterations, for any legal purpose, public or private, commercial or otherwise.

5. COMPENSATION- The Model's compensation is ______________________, to be split into

_______ even payments by the Photographer, for the amount of ____________ per each payment. The
Model agrees that the model is to be paid solely by check, cashier's check or United States Postal
Money order only, with the fees for obtaining such documents being deducted from the payment, as

necessary. The parties agree that no royalties or other ongoing compensation of any kind shall be owed
to the model under any circumstances, under any theory of law. All such compensation is expressly
waived by the model. The model understands that the model will not be compensated at all if the
photography is not reasonably completed by the model.

6. PERSONAL INFORMATION- The model (initial one ONLY) ____ DOES AGREE ____ DOES
NOT AGREE that the model's personal information may be used in connection with the photography
for reasons other than 18 USC 2257 / 18 USC 2257A. The model understands that the use of the
model's identifying information in 18 USC 2257 / 18 USC 2257A records is required as a matter of
federal criminal law. If the model has initialed the DOES NOT AGREE provision above, the model
states that the current stage name the model would like to use is:

The model understands that If he has initialed the DOES NOT AGREE provision of this clause, that
the model MUST provide a stage name. The model agrees and understands that failure to provide a
stage name will result in the use of the model's first and last name only in connection with the
photography. The model and photographer agree that identifying information other than name will be
kept by the photographer in a confidential file, to the extent permitted by applicable law.

7. COPYRIGHT- The model agrees that the photographer shall have and retain at the photographer's
discretion, any and all copyrights to the photography, per the United States Copyright Act. The model
agrees that the model has no ownership or intellectual property interests in the photography
whatsoever.

8. ASSIGNMENT- The model agrees that the photographer may sell or assign photographer's rights
under this contract at will, at any time.

9. PARAGRPAH HEADINGS- The parties understand and agree that the the paragraph numbers and
headings are for convince of reference only, and do not constitute any part of this agreement.

THE PARTIES CERTIFY THAT THEY HAVE READ AND UNDERSTAND THE TERMS OF
THE FORGOING ADULT MODEL RELEASE AGREEMENT, AND AGREE THERETO
FOREVER.
