I, GEROME M. TRIESTE, JR. , of legal age, married, Filipino, and a

resident of Poblacion, Numancia, Aklan, after having been duly sworn to in
accordance with law depose and state:

1. That I am the Notary Public who notarized the Deed of Absolute

Sale of a Portion of Land (instrument), between the vendor
Josephine M. Trinidad and the vendees Rafael B. Gregorio and
Tweeny T. Gregorio, involving a parcel of land (Mother Lot)
identified as Lot No. 3689-A, located at Brgy. Caticlan, Malay,
Aklan, with an area of ONE THOUSAND ONE HUNDRED THREE
(1,103) square meters more or less, under ARP/TD No. 05-12-
006-03267; PIN 038-12-006-01-045;

2. That said instrument was dated November 15, 2017 and recorded
under my Notarial Register with Doc No. 491; Page No. 102; Book
No. X; Series of 2017;

3. That the above-mentioned parcel of land (Mother Lot) is also

covered by Katibayan ng Orihinal na Titulo (KOT) Blg.
CARP2016001401; Patente Blg. FP-060412-15-18181;
registered on August 27, 2015;

4. That the Lot No. 3689-A, Csd-06-015051 appearing in (KOT) Blg.

CARP2016001401 with FP-060412-15-18181 as well as the Lot
No. 3689-A (Mother Lot) appearing on a the Deed of Absolute Sale
of a Portion of Land dated November 15, 2017 and recorded
under my Notarial Register with Doc No. 491; Page No. 102; Book
No. X; Series of 2017; refers to one and the same parcel of
land;

5. That I am executing this affidavit to attest to the truth of the

foregoing facts.
