Purpose: The purpose of this assignment is for you to begin to address the graduation
requirements for your major and to address the entrance requirements for your chosen four-year
institution. Please address the questions below. Please type your responses.

1. What courses are you currently taking this semester?

Education Exit Seminar (EDUC 290), GenEd-Conspiracy Theories (GNED 101), Latin
American History (HIST 251), Physical Education Elementary Teachers (HLTH 210),
Wellness Through Healthy Living (HLTH 255), and Music Methods for Elementary Teachers
(MUSI 233).

2. What courses do you need to graduate with your AA degree? (Please see Lena Paxton to get a
copy of your Degree Audit, if you have problems. Lena is located in the Matrix in the Taylor Administration
Building on the CSI campus; you can also email her at lpaxton@csi.edu.) Otherwise, go to MyCSI under
your grades option.

No, I was told by another adviser to wait until the semester is almost over.

4. What are the entrance requirements for your chosen four-year institution? What are the
application deadlines? If you plan to apply for scholarships, what are the scholarship
application deadlines? Have you applied for financial aid?

I believe I applied for FASFA, but I will have to double check to see if I did for ISU. I have
already applied for ISU for Fall 2018 and have been accepted. I am now preparing to apply
for their education program. I am unsure of the scholarship deadlines for ISU, but for NSLS
scholarshop deadlines is April 6, 2018. The entrance requirements for ISU is final transcripts
from Spring 2018, set up TaskStream (to get into education program), background check
(education program), submission of application materials: deadline March 1 or June 1
(education program), successful completion of PRAXIS I (education program), 2.75 gpa,
successful completion of Teacher Education Program Admission Interview, video for
interview (education program), copy of EDUC 202 Form, completion of the following
required classes with a ‘C’ or better: EDUC 202, 204, 205, 215., No grade lower than C and
two with grades of B or higher for the following three subjects: ENG 101 or 102, COMM
101, and MATH., the signed and notarized 2nd page of the application, Acknowledgment
Statement, indicating awareness of ID certification, student ID # for ISU, and copy of current
class schedule.

5. Does your chosen four-year institution require Praxis 1 (or other tests)? What scores are
needed? What are the dates for Praxis 1 (or other tests)? Do these tests have to be completed
before the application deadline? If so, how long does it take to process test scores?

PRAXIS score requirements: Reading 156, Math 150, and Writing 162. I have to have them
completed before the interview or if I write a petition they will allow me time to complete
before the end of Fall 2018.
