Re: HIV/AIDS Prevention and Education Coalition Letter of Intent

The PrEp for Your Future Health Clinic would like to express its interest in the forthcoming allocations from; The California
Wellness Foundation for the PrEp For Your Future Health Clinic. This letter is to inform of our intention to submit the
HIV/AIDS Prevention and Education Coalition project to The California Wellness Foundation.

The PrEp for Your Future Health Clinic has more than 15 years in providing services to the residents of San Bernardino
County. Currently the PrEp For Your Future Health Clinic administers several grant funded programs including the
American Medical Association Foundation Public Health Grants to improve vulnerable and low-income communities, in
providing free basic health care services. In addition, the California Endowment Public Health Grant will assist
underserved residents to improve access to affordable healthcare and wellness.

The California Department of Public Health, Office of AIDS, HIV/AIDS Surveillance by county in California has identified in
their surveillance summary report in 2015 San Bernardino County’s total cases of persons living with HIV infection at
3,467 cases (2.7%) and persons living with AIDS at 1,800 cases (2.6%). As a nonprofit organization we recognized our
responsibility in maintaining and improving the health of those living with HIV through our transportation services. In
addition, our clinic provides assistance in administering HIV testing and working with patients in reducing HIV disparities
through outreach and health education, counseling, treatment, and research to reduce HIV/AIDS among high-risk
residents.

The PrEp For Your Future Health Clinic will be responsible for administering the day-to-day fiscal and administrative
oversight and accountability of The California Wellness Foundation Public Health Grant. For the first year of funding, it is
reported as $1,250,201. This will contribute to administering effective rapid testing for HIV Positive patients. With an
ongoing annual funding of $3,000,000, our clinic can utilize funding for our surveillance and research program for new
vaccines and morbidity rate for HIV/AIDS patients in the county’s communities. Upon submission, the PrEp For Your
Future Clinic will provide detailed information for review and evaluation by the 22 March 2018 of submission acceptance.

We are currently interested in providing these HIV/AIDS Prevention and Education services and would like to be kept
notified of any modifications or updates related to this funding opportunity.
