1 Subcontract Agreement is based on the set of Plans & Specifications.

Subcontractor is aware that this a (3) story building and any materials/equipment will have to be taken up stairways.
2 (site has no elevator)

3 Subcontractor is aware that contract includes a payment and performance bond for total contract values over $500k.

The project duration is generally accepted to be 9 months overall and may not reflect the actual shorter duration of this
4 particular scope of work.

5 Subcontractor includes bond and MBE at 5% for the entire contract value.

6 Subcontractor includes MBE certification for the entire contract value.

Subcontractor shall Furnish & Install all Accessories for a complete system for each Flooring System, per contract
7 documents and approved submittals.

Subcontractor shall Furnish & Install all Adhesives, for a complete system for each Flooring System, per contract
8 documents and approved submittals.

Subcontractor shall Provide all Prep Work on all substrates per the manufacturer's recommendations, plans &
9 specifications for all Flooring Systems, per contract documents and approved submittals.

Subcontractor shall furnish and install all VCT, LVT and Carpet flooring in the Unit Interiors, per contract documents
10 and approved submittals.

Subcontractor shall Furnish & Install all Resilient Flooring, Base and Accessories, per contract documents and
11 approved submittals.

12 Subcontractor Shall Furnish and Install Carpet and Cushion, per contract documents and approved submittals.
 Subcontractor shall protect all work by other trades when working in a area. If working in an occupied unit, Contractor
13 shall take measures to protect Owner's Property.

14 Subcontractor has 48 hours after notification to complete punch list items.

15 Subcontractor is to Remove all trash as required for their Scope of work

16 Subcontractor shall broom sweep DAILY the area of his work inside the building

Subcontractor is to ensure ALL personnel wear all PPE at all times while on Site, to include Safety Vest, Hard Hat,
17 Safety Glasses, Boots and Hearing Protection when needed.

Subcontractor shall provide and install New floor leveling/underlayment compound to entire areas to receive VCT/LVT
18 per the contract documents, specifications and approved submittals.

Subcontractor shall sand/repair and refinish existing parquet flooring where indicated per the contract documents,
19 specifications and approved submittals.

Subcontractor shall sand/repair and refinish existing hardwood flooring where indicated per the contract documents,
20 specifications and approved submittals.

Subcontractor shall provide mock ups for each floor finishes to be applied for prior approval per the contract
21 documents, specifications and approved submittals.

Subcontractor shall provide and install stain and a minimum of (2) coats of satin polyurethane per contract documents,
22 specifications and approved submittals.

Subcontractor shall perform an inspection prior to all work to determine the reparability of existing parquet/hardwood
23 floors to see if full replacement is needed per the contract documents, specifications and approved submittals.
 Subcontractor shall provide and install carpet and pad on the townhouse stairs as indicated per the contract documents,
24 specifications and approved submittals.

Subcontractor shall provide and install all new quarter round/shoe molding where indicated per the contract documents,
25 specifications and approved submittals.

Subcontractor shall provide and install all wood reducer strips to match existing wood species per the contract
26 documents, specifications and approved submittals.

Subcontractor shall provide and install all new stair treads and landings as indicated per the contract documents,
27 specifications and approved submittals.

Subcontractor shall provide and install all Rubber base as indicated per the contract documents, specifications and
28 approved submittals.

29 Subcontractor shall provide and install new VCT at stair landings per the contract documents and approved submittals.
