UNEXPECTED SITUATIONS - 1

SITUATION 1

PILOT: Brasilia centre ANAC 123 we are experiencing severe turbulence at flight level 300. A
passenger is injured. We request return to Brasilia.

ATC: ANAC 123 roger. Turn right heading 115 and descend to flight level 240. Medical
assistance will be provided upon arrival.

EXAMINER: Now tell me in your own words everything the pilot and controller said.
EXPECTED ANSWER: The pilot reported that there was severe turbulence at flight level 300
and a passenger was injured then requested to return to Brasilia airport and the controller
told him to turn right heading 115, descend to flight level 240 and that medical assistance
would be provided upon landing.

EXAMINER: How can pilots avoid bad weather when overflying the ocean?
EXPECTED ANSWER: Pilots can avoid bad weather when overflying the ocean using the
weather radar and deviating bad weather. 

EXAMINER: Now listen situation number 2.

SITUATION 2

PILOT: Paris control, ANAC 123 I had a tail strike during take-off. We need to climb to a safe
altitude in order to check our systems before returning.
 
ATC: ANAC 123, Roger climb to four thousand feet, maintain one two zero radial from Paris
VOR. Call back for vectors to return.

EXAMINER: Now tell me in your own words everything the pilot and controller said.
EXPECTED ANSWER:  The pilot reported that he had a tail strike during take-off and that it
was necessary to climb to a safe altitude to check their systems, and then the controller told
him to climb to four thousand feet maintaining 120 radial from Paris VOR and call back for
vectors to return.

EXAMINER:  What kind of emergency or abnormal situations can a pilot face during take-off?
EXPECTED ANSWER: During take-off pilots can experience a tail strike, a rejected take off due
to an engine failure or engine ingestion.   

EXAMINER: Now situation number 3.

SITUATION 3

PILOT: Mayday mayday mayday Rio departure ANAC 123. We lost thrust in both engines. We
will try to return to Galeao airport, but we might need to ditch.
 
ATC: ANAC 123, roger. Turn left heading two three zero to land on runway one zero.

EXAMINER: Now tell me in your own words everything the pilot and controller said.
EXPECTED ANSWER:  The pilot reported that they lost thrust in both engines and they
would try to return to Galeao airport but they might need to ditch and then the controller told
him to turn right heading two three zero to land on runway one zero.

EXAMINER:  If faced with a similar situation what actions would you take?
EXPECTED ANSWER: If I were the pilot and had sufficient altitude I would try to restart the
engines because there is a specific check list for that problem.

EXAMINER:  Now, considering the three situations you heard :


- How would you compare them and which one do you think is the most difficult to deal
with? 
EXPECTED ANSWER:  IN MY OPINION THE 1ST SITUATION IS NOT SO SERIOUS.
 
THE 2ND SITUATION IS WORSE THAN THE 1ST BECAUSE A TAIL STRIKE CAN CAUSE:
THE AIRCRAFT TO BE GROUNDED FOR MAINTENANCE SERVICES DUE TO SEVERE
STRUCTURAL DAMAGE 
FLIGHT CANCELATIONS AND DELAYS 
AND FINALLY THE PILOT MAY HAVE TO BURN OR DUMP FUEL BEFORE RETURNING TO
THE AIRPORT.
 
I THINK THE 3RD SITUATION WHICH WAS A LOSS OF THRUST IN BOTH ENGINES IS THE MOST
SERIOUS OUT OF THE THREE BECAUSE A LOSS IN BOTH ENGINES CAN CAUSE:
A DISASTER
DEATHS 
AND FINALLY ITS VERY DIFFICULT FOR THE PILOTS TO RESOLVE.
