Subject: Quotation for Third Party Inspection Certification

Dear Mr. Mohammad,

We acknowledge with thanks receipt of your enquiry, with reference to the same we
are pleased to submit herewith our most competitive Quotation as detailed below:

Scope of Work
To Provide Third Party Inspection & Certification of as per relevant international
safety standards.

Rates of Inspection
The proposal rates are as follow;

Terms and Conditions:
1. All existing documents for the equipment & Trainees experience shall be made
available for the examiner / Inspector to view.
2. The client provides the test weights for the inspector‘s use and all necessary
assistance and a clean area to work.
3. Inspection/Examination shall be documented in a report/record or certificate and
be submitted within two days from the date of inspection.
4. All works shall be documented on our Work Order/ Time Sheet and client signature
will constitute the approval of both time and quality of work, these sheets will be
included as backup to our Invoices.
5. Payment should be transfer in our companies account after finish the job.
6. The client shall supply one passport size photograph for each operator along with
Iqama/ ID no. printed legibly, at the beginning of the course.
7. The client shall supply one equipment at his site to test the operators as practical.
8. Trainees / Operators, Whoever passes or Fails the payment will be due.
9. We can accept the renewal ID’S only three months after valid date.
10. Any major repair to the unit after inspection, the machine certificate is not valid
anymore and need to be re-inspected.
11. Trainees must familiar with,
A. English Language.
B. Knowledge of Calculation.
C. Technical Knowledge.
12. Other terms & conditions as per the attached appendix.
13. The client should give at least 3 days advance notice.
14. Rates shall be charge to personnel who pass or failed during the assessment, for re-
assessment (for personnel w/c failed during previous assessment service) will be SAME
unit price plus afferent transportation cost.
15. If the location is outside of the Riyadh, then client shall pay charges SR-2/km for
round trip.

 16. Working in weekends (Friday & Saturday) holidays & official holidays we will charge
overtime for our inspector per hour SR-150 regardless the equipment inspection cost.
17. Operators Manual, Load Charts, previous certification and drawings of crane and
other equipment should be available on site for review.
18. For Load test Client must supply approved load test procedure, riggers/labors,
technician other needed equipment (forklift) to load/unload test weight on site.
19. We charge 100% of the inspection/certification or assessment cost in case of failure
for any equipment or operator and repair/recertifying full fees will be applied again
for inspection or re-certifying.

VALIDITIY

Our offer is valid for a period of thirty (30) days from the date of this quotation.
