Gibson incision
Indication:
– exposure of lower third of ureter
Positioning:
– partial T burg with pt supine
1. incision: hockey stick incision extending 1 inch medial to ASIS running 1 inch above inguinal ligament, to
lateral border of rectus m.
2. divide external oblique aponeurosis in direction of its fibers (bovie, knife or kitner)
3. divide internal oblique m in the direction of its fibers
4. open transversus abdominus m.
5. retract transversalis fascia medially, carrying the peritoneum off the vessels and lateral body wall; note:
use army-navies to retract
6. locate ureter proximally as it crosses the iliacs; ureter should be lateral to retracted/reflected peritoneum
