Good Day!

I am Alexander Silvestre, a graduating student from the University of the Philippines – Diliman, currently
taking up Bachelor in Sports Science at the College of Human Kinetics. I am currently writing my thesis as “An
Investigation of the Relationship between Nature Relatedness and the Motivations behind Participation in Skin
Diving” under Alberto Dimarucut. The study aims to investigate whether there is a significant relationship between
the Nature Relatedness of recreational skin divers and their motivations behind participation in the sport. As such,
your insights will be a great contribution towards the accomplishment of this research.

Consent for Participation in Research

I volunteer to participate in a research project conducted by Alexander Silvestre from the College of Human
Kinetics. I understand that the project is designed to gather information regarding the nature relatedness and
motivations behind participation in skin diving of the research participants. I will be one of several participants
answering this research questionnaire.

1. My participation in this research is voluntary and I may withdraw at any time without penalty.

2. I understand that the researcher will not identify me by name in any reports using information obtained
from this interview, and that my confidentiality as a participant in this study will remain secure. Subsequent
uses of records and data will be subject to standard data use policies which protect the anonymity of
individuals and institutions.

3. I have read and understood the explanation provided to me. I have had all my questions answered to my
satisfaction, and I voluntarily agree to participate in this study.

4. I have been given a copy of this consent form.
