CITY ORDINANCE NO. 2011-031

AN ORDINANCE AMENDING SECTIONS 5 AND 6 OF THE


CITY ORDINANCE NO. 2011-001, “AN ORDINANCE
REGULATING THE GRANT OF MEDICAL/MORTUARY
ASSISTANCE FOR THE INDIGENT MEMBERS OF THE
FEDERATION OF SENIOR CITIZENS ASSOCIATION OF THE
PHILIPPINES (FSCAP) – CABADBARAN CHAPTER,
CABADBARAN CITY, AGUSAN DEL NORTE”

SECTION 1. Section 5. Amount of Coverage of CITY ORDINANCE


NO. 2011-001, which reads as :

a. In case of outpatient due to sickness or accident, the concerned


senior citizen shall be given financial assistance corresponding to
the amount of the medicines required, but not to exceed Five
Hundred Pesos (P 500.00) which shall be availed once a year.

b. In case of hospitalization due to sickness or accident, the concerned


senior citizen shall be extended financial assistance corresponding
to the amount of medicines prescribed by the physician and other
incidental cost while confined in any hospital, but such amount shall
not exceed One Thousand Five Hundred Pesos (P 1,500.00) within a
calendar year.

c. In case of death, the legitimate beneficiary concerned of the


deceased indigent senior citizen shall receive an amount of One
Thousand Pesos (P 1,000.00) as burial assistance from the City
Government.

IS HEREBY AMENDED TO READ AS FOLLOWS:

SECTION 5. AMOUNT OF COVERAGE –

a. In case of outpatient due to sickness or accident, the concerned


senior citizen shall be given financial assistance corresponding to
the amount of the medicines required, but not to exceed One
Thousand Pesos (P 1,000.00) which shall be availed within a period
of one year.

b. 1. In case of hospitalization due to sickness or accident, the


concerned senior citizen shall be extended financial assistance in
the amount of One Thousand Five Hundred Pesos (P1,500.00) to
be given in full within a calendar year for medicines and other
incidental cost while confined in any hospital and after his/her
release.

2. For those PHILHEALTH beneficiaries, they can still avail the


assistance when the bill exceeds the PHILHEALTH coverage and
in cases where there are prescribed take home medicines.

c. In case of death, the legitimate beneficiary concerned of the


deceased indigent senior citizen shall receive an amount of One
Thousand Pesos (P 1,000.00) as burial assistance from the City
Government

SECTION 2. Section 6. Requirements to Claim Medical/Mortuary


Financial Assistance of CITY ORDINANCE NO. 2011-001, which reads
as:

6.2. For hospitalization due to sickness, or accident, the concerned


hospitalized senior citizen himself or his/her authorized
representative shall claim the amount corresponding to the
medicines prescribed by the physician and other incidental cost
while confined in any hospital, but such amount shall not exceed
One Thousand Five Hundred Pesos (P 1,500.00) within a calendar
year from the City Treasurer’s Office upon presentation of the
following:

6.2.1 Duly signed Hospital Billings;

Section 6. REQUIREMENTS TO CLAIM MEDICAL/MORTUARY FINANCIAL


ASSISTANCE

6.1 x x x x

6.2 For Hospitalization due to sickness or accident, the concerned


hospitalized senior citizen shall be extended financial
assistance in the amount of One Thousand Five Hundred Pesos
(P 1,500.00) to be given in full within a calendar year for
medicines and other incidental cost while confined in any
hospital and after his/her release from the City Treasurer’s
Office upon presentation of the following:

6.2.1 Physician’s certification;

SECTION 3. This Ordinance shall take effect in 2012.
