On the following pages list the musical selections the musician/group wish to perform and fill the
required information for each selection. One song per page. Minimum of 2 songs up to maximum of 5
songs.

MUSIC SELECTIONS – List ONLY the music selections you want to be considered by the Music
Committee. Send a minimum of 2 songs from the 4 topics to be given by the NCMC MAS-AMICUS
Music coordinator and a maximum of 5 ONLY and list your choices (first choice, second choice, and so
on). This does not mean that all music selections you send in for consideration will be accepted for
performance at mas amicus convention. The NCMC Mas Amicus Music Committee (NCMC MAMC) can
review numerous selections and choose what is suitable for the day and program. Applicants will be
notified by their District Music coordinator of the songs selected and approved for performance after
the NCMC MAMC has made those decisions. Please fill out one form for each song you want to be
considered.

Performance diagram (p) indicating the typical staging for all musicians/groups for each song to
