myself and the peers present at clinical and my instructor.

Judie was clear and direct. She was present for shift handoff to be informed about client changes and revenant information. Judie used
Point Click Care to document progress notes for client care and signed of medication and treatments. Using FDARE she was able to
organize assessments, interventions and follow up of care.

(2). This resident understood and remained still in order to not contaminate a sterile field.

Judie demonstrated therapeutic communication through using acting listening, touch, and being aware of verbal and nonverbal cues of
clients. The client demographic at Villa Leonardo Gambin was mainly Italian, Judie made an effort to learning basic words to enhance her
communication with clients. She respected the beliefs of clients, family, staff and her peers.

occasions including medication administration, PPE situations, injections, etc.

and positive expected outcomes were also met.

Head to toe assessments were done on a weekly basis, but I was also able to make sure that their needs were met throughout the day: such as

toileting, making sure that the resident attended their meals, provided them with company, etc.

J.C. : I believe that I was able to provide information that contributes to the evaluation and refinement of plans of care (4). For example, when

assessing lab values (3) on my last resident, I noted that her TSH level was higher than the normal range, which would indicate hypothyroidism;

however upon further research her DX of lewyy body dementia would indicated an elevated TSH level. Extra information and research like this could

contribte to the refinement of plans of care.

(4). For example, I only perform the tasks and care that is within my scope of practice (have been trained), such as changing eg tube feed,

medication administration, wound care, injections, etc.

map that helped me decide what nursing care is appropriate for the resident that I am caring for. For example, A resident that I cared for with

dementia responded well to music therapy, so I used this intervention when she showed signs of pain.

done before breakfast, and I am able to keep my head to toe assessments within 10 minutes.

Judie completed preparatory work for lab and completed client research on diagnoses, medication and treatment prior to attending clinical.

were working with in order to allow the nurse to take vitals on the resident that had fainted.

example, one of the resident was visibly upset with the change in dining room seating, so I consulted with some of the PSWs who worked with that

resident. The end result was that the seating in the dining room would be re arranged.

unique role that the nurses have is access to medication, whereas PSWs did not.When giving medications, We had to thoroughly communicate with

the nurse that was administering the medication .

Judie worked in a team meeting with peers, PSWs and registered staff. She reviewed client's care plan and created a concept map to

decisions that have purpose and reasoning behind it. For example, When a PSW insisted that safety straps were not needed to be used because she

"knew her resident" I intervened and the reason was because it is policy to use them and ensure the safety of the resident.
