M.H.D. : I demonstrated this skill through showing up on time (before 0700), having all paperwork completed, and wearing gloves when cleaning the
patients and making beds. I also perform 3 checks of medication administration.

M.H.D. : I prepared all the necessary paper work prior coming to placement: medical research, diagnosis research, and daily care plans for the
assigned residents each week. There was one week that I did not act responsible, which was in week 6. I forgot to bring the required document to
placement. I was sent home to get it, but I made arrangement with my aunt as she was bringing it to me instead.

M.H.D. : My strengths are that I am confident to initiate conversations with the residents. Moreover, I am able to administered medications to the

assigned residents accurately. I can also provide am care to the assigned residents comfortably with the help of the PSW. I am able to help the PSW
in moving/cleaning/transferring residents.
One of the limitations that I need to improve on is that I need to learn more thoroughly different kinds of medications to ensure I give the correct

medications for the corresponding conditions. Another weakness that i noticed is that I do not know how to deal when situations changed. I need to
review my notes and re-watch nursing videos to make sure I am familiar with some caring processes.

I seek assistance from my teacher when i noticed one the my assigned residents being moody and weak. It turned out that she was hypoglycemic,

that was why she acted like that. My teacher then told the nurse about the situation. the nurse gave the resident apple juice to bring up her glucose
level.

M.H.D. : When communicating with the residents, I always use elder speak to talk to them. I make sure that I speak loudly, slowly, clearly with a low-

pitched voice. This strategy is from what I have learnt in theory class. I am able to apply the knowledge I learn in class to practice.

M.H.D. : I make sure that the residents I am taking care for have non-skid socks, shoes, and eyeglasses on. I make sure their room is free of

obstacles so they will not trip on them and fall. After making their beds, I always adjust the bed's height to its original position (some beds are in low
positions, some are at waist level).

M.H.D. : I ask the PSW how some residents transferred to ensure that I would not do something that is dangerous to them. For example, I asked the

PSW and found out that some residents transferred by the standing lift. Therefore, those residents have limited mobility. When I care for those
residents, I make sure to call the PSW to use the standing lift to transfer them.

M.H.D. : I understand the role of the nursing student. I help the PSW in doing morning care for the residents and transfer them. I need to check the
medications with my teacher before I administer them to the residents. I ask the nurse how some residents take their pills to make sure i give the
correct route. If there is any unsafe situation, I report immediately to the nurse, the PSW, or my teacher.

M.H.D. : After I administer medications, I do not forget to document using a computer. I document right after i administer the medications and before I
leave the floor for breaks.

M.H.D. : I asked the nurse how a resident takes her pills. The nurse told me that the resident takes crushed medications with apple sauce. I ask

question about medication to the appropriate nurse, not the PSW.

M.H.D. : I use one of the PSW's or my teacher's account to research my assigned resident care plan. I also use the kardex to get their information. I
use evolve textbook and drug book to research my residents' conditions and medications.

M.H.D. : I am aware and understand about myself. By doing that, I can build rapport, trust, and empathy with my residents. For example, I know that I

like to smile with people I see, so every time I walk by a resident, I smile at him/her. Moreover, when I am talking to a resident in her room, I always
smile, knot my head, and ask how her day is going. These actions help me to establish relatedness and build trust and therapeutic relationship with

my residents. The result is the residents feel welcoming, and ask me to come back to their rooms sometimes.

M.H.D. : I am aware of myself, my personalities, beliefs and values. I understand that these elements contribute to the way i care for clients. I believe

that everyone has his/her own choice and I need to respect that. When caring for a resident, i always ask if it is alright to do a certain job. If she

refuses, I will not do it.

M.H.D. : I did not really educate any resident much. All I did was reminding them to wear non-skid shoes to prevent falling. I need to review this

M.H.D. : One time, I was bringing in lunch for one of the residents in her room. I told her that I am a student nurse and that I was helping her with her

lunch. She told me that she wanted a nurse and did not let me to set up the table for her. I asked her again if I could do it instead and she refused. I

respected her request and told the nurse/PSW to come.

M.H.D. : There was one time after our lunch break, we had nothing to do. My peers were sitting in the resident lounge chatting. I thought that I would

not look nice if we sat there doing nothing. So I told them that we should ask the PSW/nusre if they need help, or at least do something productive.
They all agreed and we actually helped the PSW some work and chatted with the residents.This demonstrates fostering group work.

M.H.D. : I always ask the clients if it is alright to do certain work/assessments. If they have no problem, I will go ahead and do what I need to do. If

they refuse, I respect that and find out alternatives with them so we can both agree.

M.H.D. : There was one time that I fell asleep in the resident area and was reported to my teacher by one of the staff. I recognize and accept my
mistake. I accept that it was an unprofessional behaviour. I will never do it again.

M.H.D. : When doing the concept care map, I identified the resident's problems and needs. I developed nursing interventions to help her improve her

conditions. I also set out goals and expected outcomes if her conditions got better.

M.H.D. : When doing the concept care map, my plans of care were holistic and my expected outcomes were client-centered. For example: "The client

M.H.D. : When doing the concept care map, I use knowledge, skill and judgement to assess the resident. For example, if she has Alzheimer's

disease and is quite, I would try to talk more to her to provide company. Moreover, I prioritize needs such as I know that she is at risk for loneliness,
that's why I talk to her a lot. The outcomes are that she will be more socialize and enjoy activities.

M.H.D. : I safely perform vital signs, neurological and cardiovascular assessments on some of my residents. I know that some assessments are hard
to perform on the residents or not relevant to their conditions that might cause danger to them. Moreover, i know that some assessments such as the
MSK is not within my skill abilities to perform on immobile residents. Therefore, I refine appropriate assessments and interventions to the
corresponding conditions.

M.H.D. : I safely perform vital signs, neurological and cardiovascular assessments on some of my residents. I know that some assessments are hard

to perform on the residents or not relevant to their conditions that might cause danger to them. Moreover, i know that some assessments such as the
MSK is not within my skill abilities to perform on immobile residents. Therefore, I refine appropriate assessments and interventions to the
corresponding conditions.

M.H.D. : For instance, I know that the resident I was taking care of has stroke, atrial fibrillation, and some complications of type 2 diabetes. Therefore,
i decided to perform cardiovascular and neurological assessments on her. I did not choose MSK or respiratory because they are not relevant to the

conditions she has. This demonstrates decision-making framework.

M.H.D. : I demonstrated teamwork skill through motivating my team to do some productive work. Usually, we do not have many jobs to do after lunch

break. my teammates like to sit on the chairs in the resident's lounge. I thought that it is not the right thing to do because we were warned by our
teacher not to hang around. I told them they we should do something otherwise the teacher came suddenly and we would be in big trouble. They all
agreed and we actually did some productive jobs such as helping the PSW.

M.H.D. : I was supposed to administer medications and eye drops before a resident's dinner. Unfortunately, the resident was already at the dinning

table and I could not administer eye drops at the dinning table. I gave her the medications and told her that I needed to move her to the nursing station
to give her the eye drops. She refused and told me to do it at the dinning room. I told my teacher about the changing situation and my teacher
suggested to administer eye drops after her dinner at the nursing station. Later on, the resident called me over and told me to do the eye drops right
away because she had someone visiting her after dinner. I told her that the eye drops would not take long and that I would do it after she ate. She
agreed and I successfully administered eye drops for her.

M.H.D. : I respect each other's choices and actions. Because it is based on their knowledge, beliefs, personalities, and cultures. If my peers want to

do differently than what I wanted to do, I will listen to their rationale. If their choices seem reasonable, I would let them do it and follow them to see
how they do it. Because I might learn something new from my peers. If their choices are completely opposite to what I think or I know that the choices
are wrong, I would softly explain to them with respect and hope to find out the right solutions.

M.H.D. : I act in the best interests of clients and protect them from harms. For instance, when i was doing morning care for a resident, I found out that
her denture was not in the container or anywhere in the bathroom. I looked in her bed, around her room many times but could not find it. Finally, I

decided to reach out for my teacher and the PSW on that side. They helped me to find the denture. Otherwise the resident would be at risk at
aspiration if she ate without her denture. Eventually, the PSW found out that the denture was in the resident's mouth the whole night. I forgot to check
her mouth. Therefore, I collaborate with other members in the inter-professional health care team to provide safe care for my resident.

7. 2 Participate in self-reflection to identify strengths, areas for improvement. Satisfactory Satisfactory

M.H.D. : I did participate in self-reflection to identify strengths and weaknesses. I handed in my self-reflection to my clinical teacher on time.
My strengths are that I am confident to initiate conversations with the residents. Moreover, I am able to administered medications to the assigned
residents accurately. I can also provide am care to the assigned residents comfortably with the help of the PSW. I am able to help the PSW in

M.H.D. : I have created a learning plan about areas I need improvement on. I need to review the textbooks as well as online videos about caring for

preoperative and postoperative patients; different types for dressing change; nursing actions in blood transfusion; how to deal with
difficult/uncooperative patients; and safe transfer patients using different devices. I try to learn as much as I can. This summer I will spend time to
review my nursing notes. Hopefully by the start of the third semester, I will be more confident approaching new complicated patients with a full brain of

knowledge I learnt from the first year.

M.H.D. : Whenever I get a chance to help to PSW transfer/bath/clean a resident, I always watch carefully and sometimes ask questions to foster

learning. I am a visual learner, so by watching someone does a certain job over time, I could learn fast from that. While watching the PSW do their job,
I also rationale in my head why she does this way. This helps me a lot in understanding care and learn faster. For instance, I watched my teacher
administered eye drops to one of the residents, I watched her techniques and wordings. Therefore, when it was my turn to do it, I did exactly to what
she showed me, even more effective than her method.

M.H.D. : I learn from other health care professionals, from my teachers, my peers, textbooks, online videos, learning modules to improve nursing
practice.

M.H.D. : There was one time I did not use problem-solving skills to inform decision making to my teacher. It was in week 6 when i forgot to bring the
required document and was asked to return home to get it. I called my aunt and she agreed to bring to document for me instead. I did not
communicate with my teacher about the changed plan. The result was that I got unsatisfactory for this criteria that week. In general, I need to utilize
problem-solving skills in all aspects of nursing care.

M.H.D. : I wrote a self-reflection identifying my strengths and weaknesses. However, I know that I need to improve problem-solving and decision-

making skills as well because these skills are essential in nursing practice. My plan to improve these skill is to think critically every time I overcome a
difficult situation. I should watch more nursing videos and read about how to deal with complex situations. The best practice would be experiences.
The more I interact with clients, the more improvement in these problem-solving skills.

M.H.D. : I prioritize task for client care. Between medications and eye drops, I know that I should administer medication first because it is more

important than eye drops. It cannot be missed but eye drops could. However, I still need more improvement in this criteria. I still have limited
recognizing of which task should be taken first.

M.H.D. : I need more improvement in this criteria. One time I was found sleeping in the resident's area by one of the staff. It effects my

professionalism. Other than that time, I act professionally in caring for the residents. I show up on time, use therapeutic communication with the
residents, complete all paper work for clinical, and have all necessary equipment to perform assessments.

M.H.D. : My strengths are that I am confident to initiate conversations with the residents. Moreover, I am able to administered medications to the
assigned residents accurately. I can also provide am care to the assigned residents comfortably with the help of the PSW. I am able to help the PSW
in moving/cleaning/transferring residents.

One of the limitations that I need to improve on is that I need to learn more thoroughly different kinds of medications to ensure I give the correct
medications for the corresponding conditions. Another weakness that i noticed is that I do not know how to deal when situations changed. I need to

review my notes and re-watch nursing videos to make sure I am familiar with some caring processes.

I seek assistance from my teacher when i noticed one the my assigned residents being moody and weak. It turned out that she was hypoglycemic,
that was why she acted like that. My teacher then told the nurse about the situation. the nurse gave the resident apple juice to bring up her glucose
level. Not only that, I will seek assistance from my teacher/nurse/PSW whenever I am unsure because I want to minimize mistakes in the health care
settings.

I need more development on problem-solving and decision making skills. I need to review the textbooks as well as online videos about caring for
preoperative and postoperative patients; different types for dressing change; nursing actions in blood transfusion; how to deal with
difficult/uncooperative patients; and safe transfer patients using different devices. I try to learn as much as I can. This summer I will spend time to
review my nursing notes. Hopefully by the start of the third semester, I will be more confident approaching new complicated patients with a full brain of
knowledge I learnt from the first year.

You are practicing at a beginning practitioner level. You can competently care for two residents including medications administration, personal care
and health assessments. Please utilize the open access lab during the summer to practice your skills ie dressing changes, vital signs, s/c and IM
