Introduction
How knowledgeable is that FDA inspector standing in reception area? He or she may be conducting
their first solo inspection to achieve Level I certification or may be a senior, elite investigator, such as
the members of the Pharmaceutical Inspectorate.
While they all have the same authority once they’re sent out by the agency to conduct solo inspec-
tions, they are not all equally trained. The typical inspector will advance through several stages of
professional development before reaching the peak of their career. Understanding that training may help
you better handle your next inspection.
Though industry refers to those who audit pharmaceutical and device plants as “inspectors,” the FDA
uses the term differently. An “inspector” is a lower-level employee who collects samples, while the “in-
vestigators” are those who have the responsibility for conducting the inspections and identifying problems
and violations, explains Robert Coleman, senior compliance expert for pharmaceuticals at IHL Consulting
Group. Coleman spent 35 years with the FDA, 13 of those as a National Drug Expert Investigator.
While that is a key duty, the official job description of a “consumer safety officer,” which is how in-
vestigators are known within the agency, lists an array of duties for individuals hired for these positions:
• Investigation of complaints about an FDA-regulated product;
• Initiation of actions against violators;
• Advising industry, state and local officials and consumers on enforcement policies, methods and
interpretation of regulations;
• Planning and directing regulatory programs;
• Developing inspection procedures and techniques; and
• Coordinating the review process of new product applications.
Educational requirements, according to the FDA job description, are:
“A full course of study at an accredited college or university leading to a bachelor’s or
higher degree, including 30 semester hours in one or a combination of biological sciences,
chemistry, pharmacy, physical sciences, food technology, nutrition, medical science, engineer-
ing, epidemiology, veterinary medical science, or related scientific fields that provide knowledge
directly related to consumer safety officer work;
Or 30 semester hours of course work as described above plus appropriate experience or ad-
ditional education. The required 30 semester hours can include up to 8 semester hours in statis-
tics or course work that includes the principles, theory, or practical application of computers or
computer programming. To qualify for higher-graded positions, candidates must have additional
amounts of either specialized experience or directly related education.”
Consumer safety officers, or investigators, may be located at FDA headquarters in Maryland or
at district offices located throughout the U.S. As of the end of 2012, the Office of Regulatory Affairs
(ORA) had a total of 1,668 investigators on staff. That figure includes field investigators, regional inves-
tigators and headquarters-supervised investigators. An FDA spokesman notes that many investigators

do conduct inspections in more than one regulated product area, making it difficult to break down how
many specialize in drugs versus devices versus biologics, for example.
For the most part, new investigators go through the same general process to start. Timothy Wells,
founder of the regulatory consultancy QualityHub and a former FDA investigator, noted that there can
be some variation among districts, but the overall procedure is the same. Wells spent 24 years at the
FDA, 10 with CDRH.
However, some new hires may come to the FDA with significant background in a specific program
area; these may be more likely to be directed to that area as quickly as possible, Coleman says. An
example might be an individual who was hired from industry or from elsewhere in the Public Health
Service, he says. But transfers from industry are rare, despite occasional FDA efforts to draw people
from industry into investigator positions, primarily due to the salary differential between industry and
government work, he adds.
According to the FDA website, the starting salary for a newly hired investigator is a GS-5 (a base
of approximately $27,500/year, with some variations by geographical location), although some for-
mer agency investigators noted that it is possible for some individuals to start at GS-7 (base of about
$33,900/year). At the field level, pay ranges through GS-11 (base of about $50,200/year), primarily by
experience and continuing education. The consumer safety officer job description further states:
“Higher-grade levels in both headquarters and field offices are available based on peer re-
view of individual accomplishments or supervisory responsibilities. Higher graded positions are
directly related to additional education and/or specialized experience.”

Basic Training (Level I Investigators)


Regardless of whether investigators ultimately end up focusing on drugs, devices, biologics or an-
other area, all investigators start out as generalists.
“We weren’t allowed to specialize for several years,” Wells tells FDAnews. “There is a whole
process of learning the law, learning evidence development and gaining investigation skills. Often, the
investigators started in basic food inspections and worked their way up to more complex products and
more complicated manufacturing operations.”
Specialization can occur more quickly now than in years past, according to FDA training documents.
But new investigators still must go through the agency’s initial training program, which focuses on basic
food and drug law, evidence development and personal safety.
While this training once involved almost exclusively face-to-face courses held either at district of-
fices or a national training site, the FDA has in recent years shifted more of its training to on-line cours-
es, as is highlighted in the agency’s course catalog. You may view the course catalog at http://www.fda.

Coleman and Wells agree that classroom education has certain advantages, including the opportunity
for new and experienced investigators to share their own unique perspectives and experiences, as well as
for questions asked of instructors to generate useful discussions.
Instructors for both web-based and face-to-face training courses come from academia, industry and
government. The inclusion of industry experts provides important real-world scientific and engineering
perspective, Coleman says.
While at one time testing was not required for certification, FDA training courses are now accredited
through the International Association for Continuing Education and Training, which requires a way to
assess participants’ knowledge and application of that knowledge.
New investigators also go out with senior investigators on “training audits” to observe FDA procedures
and policies in action. These are regular inspections for the drug or device manufacturer; the senior investi-
gator conducts the inspection as usual while the trainee accompanies him or her and observes.
Completion of this basic training certifies an investigator at Level I, which Coleman calls the “be-
ginner” level. Once they have undergone the basic introductory training, investigators may begin their
studies to specialize in a particular program area, such as drugs or medical devices. Prerequisites for
specialized training include:
• Completion of web-based training curriculum;

• At least six months of employment as an FDA investigator;

• Independent performance of at least one establishment inspection (if working in a remote loca-
tion, a team or training inspection may be substituted);

• Completion of district laboratory tour/on-the-job-training experience; and

• Completion of any pre-course reading assigned by the FDA’s training branch prior to the course.

Certification to Levels II (which Coleman calls the “journeyman” level) and III (the “expert” level)
may take place in any of the FDA program areas, such as drugs, medical devices or biologics; it is at this
level that investigators may begin to specialize. Investigators also must apply and be approved for spe-
cialization. Completion of certain courses, largely on-line, and on-the-job training under a senior investi-
gator who specializes in the particular area, are required for Level II certification.
“Generally, it was a four-year process to get up to a grade that could even qualify the investigator to
consider specializing and being really proficient, in my opinion,” Wells says. For instance, in the 1970s,
FDA investigators underwent a lengthy apprenticeship program, approximately three years, he says.
“There were six months of on-the-job training, a combination of classroom and hands-on work with
senior investigators. Investigators were not allowed to do solo inspections until after they had proven
themselves capable,” he says. “From what I see now, it appears that the investigators are pushed out
onto solo, and often complex, inspections earlier in their careers.”
Coleman agrees with that observation, noting that, during his tenure at the agency, investigators un-
derwent an intensive six-month training period covering all inspectional aspects at a rudimentary level,
and work was assigned progressively, as investigators gained experience. For instance, a new investi-
gator might begin with learning how to handle consumer complaint investigations and how to collect
samples for analysis, often starting with simple food warehouse inspections.
“Once they master finding and documenting violations, they would get more responsibility,” he says.
“Then the investigators can move into other program areas, such as pharmaceuticals, where they would
start by taking basic training courses and going out on inspections with a more experienced investigator.”
But while the timeline may have changed somewhat, the progression from Level I generalist to a
Level II or III investigator specializing in medical devices, pharmaceuticals or another regulated product
follows essentially the same path.
Specialization in drugs or devices depends primarily on which industry dominates in the district to
which a fledgling investigator is assigned, Coleman says.
Wells agrees that the needs of a particular district tend to have the greatest influence over what spe-
cialties a particular investigator may develop. The Chicago area, for instance, is home to many phar-
maceutical and medical device manufacturers, so there is a need for more drug and device specialists in
that district than some other districts. On the other hand, California has probably the greatest number of
medical device companies, so their need for device specialists is quite high.
However, Coleman also notes that a new investigator’s background may also predispose him or her
to a particular area of specialization. New or experienced investigators alike may ask to work in a spe-
cific program area or particular location, Coleman says. But whether such requests are granted depends
ultimately on where the FDA’s needs lie.

Drug-Specific Training (Levels II and III)


For the pharmaceutical program area, the specialization process begins with a 10-day course the
FDA has titled “Basic Drug School,” which provides basic training in cGMPs and drug quality issues.
Taught by FDA and industry experts, the class includes technical sessions on various types of drug
manufacturing and a facility tour. Upon completion, participants are expected to prove their ability to:
• Describe basic principles of drug manufacturing, quality control and quality assurance practices;

• Outline a systematic inspectional approach to assess a firm’s compliance with cGMP regulations;

• Identify evidence to support inspectional findings; and

• Identify FDA, industry and academic resources pertinent to drug manufacturing.

Related courses include one on systems-based drug inspections, which consists of a series of web-
based learning activities, a two-week classroom session and a field experience. It includes further educa-
tion about applying GMP requirements and guidances during inspections, focusing on quality systems,
facilities and equipment, materials, production system, packaging and labeling, and laboratory controls
inspections. Participants must demonstrate their ability to discuss regulations and approaches to systems-
based inspections, including use of automated systems and electronic data, and risk management.
This course also covers issues specific to inspecting API plants, including how regulatory expecta-
tions differ from finished dosage manufacturing. Other topics covered—and tested—include:
• Roles and responsibilities of field personnel and the centers for pre-approval inspections;

• Recognizing the significance of inspectional findings and how they relate to field recommenda-
tions to approve a product; and

• Principles of industrial sterilization, lyophilization, aseptic processes, removal of bacterial endo-


toxins and supporting utilities, including equipment, methods and validation.

Though the systems-based inspection course touches on pre-approval inspections, the FDA also pro-
vides separate training specifically on this type of inspection, via a five-day course that consists of lectures,
case study presentations and workshops. Similar classes are available for more in-depth training in:
• API manufacturing (a prerequisite for investigators pursuing Level II certification in the drug
program);

• Pre-approval and sterile APIs;

• Biologics manufacturing;

• Process validation for drug manufacturing operations (also a Level II prerequisite);

• Process analytical technology (class participants screened by CDER or Office of Regulatory Af-
fairs (ORA) before acceptance); and

• Biomanufacturing principles and processes.

The Level III certification training program is designed to add to the skill sets of investigators who
routinely conduct complex drug inspections. Level III certification represents an investigator who is
considered expert in his or her program area and district. As with previous levels, the requirements are a
mix of courses to take and professional experience to collect, Coleman says.
This program includes such topics as risk assessment, pharmaceutical quality, quality by design
(QbD) and design of experiments (DOE). Discussions of transdermal and metered-dose inhaler drug
products, as well as positron emission tomography (PET), are also included.
Preference for attending the Level III training course is given first to investigators who are already
Level II drug-certified, then to those who have submitted their requirements for Level II certification and
lastly to those who have completed some of the key drug training courses—Basic Drug School, pre-
approval inspections, API manufacturing, process validation and industrial sterilization.

Device-Specific Training (Levels II and III)


For investigators seeking to specialize in medical device inspections, the first steps include com-
pleting 11 on-line courses on the Quality Systems Regulation (QSR), five on the Quality Systems
Inspection Technique (QSIT) and one entitled “Step-by-Step Approach to Process Validation.” They
also must conduct at least one inspection of a medical device manufacturer; training inspections count
for this requirement.
Once these prerequisites have been met, an investigator is eligible to take the FDA’s Basic Medical
Device course, which covers medical device laws, regulations and policies and how they are applied to
inspections. Participants are expected to prove their ability to evaluate common device manufacturing
processes and the control systems used to ensure QSR compliance, as well as to apply investigation and
evidence development techniques to device inspections.
Another important device-related course covers process validation of medical devices, including
key elements of validation processes and how the process validation concept may apply to packaging,
electronic manufacturing and assembly, metal fabrication and testing, and IVDs. Investigators must have
completed the Basic Medical Device training course and its prerequisites before taking this one.
Testing after this course is designed to show that the investigators can apply process validation
principles and QSR requirements during a medical device inspection, including demonstrated ability to
evaluate how process validation is carried out for manufacturing processes and whether a firm has cor-
rectly validated its processes.
The FDA also offers a device-specific course on electronics, computers and materials, which is
intended to improve experienced investigators’ ability to identify potential regulatory issues related to
the design, manufacture, testing and quality assurance for medical device electronics, computers and
materials. Investigators must have at least five years of experience in conducting inspections, the major-
ity of which must have been done in the device arena. They also must have completed the Basic Medical
Device and process validation courses.

Beyond Level III: Continuing Education


Achieving Level III certification is far from the end of an investigator’s education. The FDA pro-
vides a wide variety of continuing education courses, all based on different specialties, such as medical
devices, blood banking, drug manufacturing and microbiology, among many others. Promotions require
attending these courses, Wells says. In addition to the formal in-house training offered by the FDA, con-
tinuing education may include attendance at recognized meetings that issue continuing education units
(CEUs) or instruction from such outside sources as universities, Coleman says.
Training may be investigators’ current area of expertise, or they may seek cross-training in other
program areas, an FDA spokesperson says.
Coleman notes that part of his own continuing education was a trip to the FBI training academy to
learn interviewing skills to better talk to different people and get them to provide the information need-
ed, something he says is also an important skill for any FDA investigator.
Other important training that may come from outside the FDA includes learning situational respons-
es for when things are less than calm, Coleman notes.
FDA investigators also get training on new GMP guidances as they are developed for their areas of
expertise, Coleman says. This training will occur before the guidances are finalized to ensure consistent
application, an FDA spokesperson says, noting that since guidances generally express existing agency
expectations and practices, the investigators’ day-to-day activities and observations play a significant
role in developing new or revised GMP and QSR guidance documents.
“There is also cross-training,” notes Wells, who worked in several locations over his 24-year FDA
career. “If you worked in Chicago, for instance, FDA might encourage you to spend some time working
in a resident post or in another district office to gain knowledge of other companies and other districts.”
Investigators who achieve Level III certification may also pursue additional training that may allow
them to join elite groups of experts, such as the Pharmaceutical Inspectorate on the drug side or Team
Biologics in the biologics arena. The goal of creating these sorts of highly specialized groups was to
provide the best and brightest investigators—usually no more than one or two per district—access to
advanced training where it exists, Coleman says.

Combination Products
Combination products can pose a challenge to investigators. Inspections of facilities that make these
products or their components might call for a team of investigators with specialized knowledge in the
different components, Coleman says. The FDA will pull in inspectors from other districts if needed.
“Most district offices do have the needed expertise somewhere,” Coleman says. “But that is not
always the case. For instance, Alaska might not have pharmaceutical experts, but they could send to
Seattle if they needed someone for that work.”
Wells adds, “FDA has qualified investigators in pharmaceuticals, medical devices and biologics. The
way the combination product GMPs are written, you would need knowledge of more than one of those
areas to properly inspect for the combination product GMPs (21 CFR Part 4).”
However, he adds that the high workload at the FDA usually means that no more than one senior
investigator, accompanied by two to three junior investigators, is assigned to any given inspection. It
would be costly, in terms of workload, to send a device and a drug expert to a single inspection except in
rare cases where serious compliance issues were in play.
Combination products are governed by just one center (CDER, CDRH or CBER), depending on how
they were classified during the approval process. Generally, a senior investigator who is experienced in
dealing with the products governed by the lead center will be assigned to lead inspections of the manu-
facturing facility. However, outside expertise may be called in as needed.
Coleman notes that there is a group that works for FDA headquarters, though they may be assigned
to various districts, known as the national expert staff. These experts can be tapped if a district needs
particular expertise for an inspection and does not have a local investigator with that skill set.

Foreign Inspections
Foreign inspections also may pose special considerations. Investigators that conduct inspections out-
side the U.S. first must be qualified to do inspections stateside, and at least Level II-certified. Coleman
notes that the FDA has a voluntary program that allows interested and qualified investigators to join the
International Inspection Cadre for a period of time, usually two years. During this time, all their inspec-
tions are done outside the U.S. and, although they work from their own district offices, they answer
directly to FDA headquarters.
Training for foreign inspections is not very different from that for U.S. inspections, because the laws
and requirements are the same for any product sold in the U.S., regardless of the location of the manu-
facturing plant. However, some additional non-technical training is provided, including how to handle
issues that may arise regarding overseas travel or dealing with local customs.
The FDA avoids sending people to unsuitable locales, Coleman says. For instance, female investiga-
tors are rarely, if ever, assigned to inspect facilities in many Middle East nations.
Coleman notes that in the 1970s, an investigator could not even apply to join the International Inspec-
tion Cadre until he or she had spent a couple of months overseas with an expert, who then rated the inves-
tigator as capable or not. Now, investigators do not have to prove their capability overseas before receiv-
ing training and applying to join the foreign inspectorate. They do, however, have to have demonstrated
proficiency in conducting stateside inspections in their program area (drugs or devices, for instance).
