MANUAL OF OPERATION PROCEDURES AND GUIDELINES FOR


PRACTICUM

1.) The choice of Cooperating Agency is primarily the


responsibility of the Adviser and the Department Head.
The students concerned can recommend a Cooperating
Agency, approval of which is also the responsibility of
the Adviser and Department Head.

2.) The practicum students shall provide the Agency the


following documents, receiving copies of which must be
submitted to the Adviser.
a.) Endorsement letter duly endorsed by SABM Dean,
and noted by the Department Head.
b.) Application letter of the practicum student.
c.) Curriculum vitae/ Personal Data Sheet of the
student.
d.) SABM Manual of Operation Procedures and
Guidelines (MOPG) for practicum.
e.) Memorandum of Agreement (MOA) between the
University and the Cooperating Agency.
f.) Other documents may be submitted as required by
the Cooperating Agency or the Department Head.

3.) The grading system per period will be based on the


following:
a.) Work plan (for prelim only and if required by
Cooperating Agency).
b.) Daily Time Record/ DTR duly summarized and signed
by immediate Supervisor/s in the Cooperating
Agency.
c.) Progress report/ insights/ learning.
d.) Individual journals countersigned by immediate
Supervisor/s.
e.) Duly accomplished evaluation by immediate
Supervisor/s. The evaluation must be duly signed
and sealed in an envelope and immediately
submitted to the Adviser.

The grading system shall be as follows:

4.) Absences and Tardiness shall be bound by Cooperating


Agency Policies but students are required to inform their
immediate Supervisor/s and Adviser of their absence.
Excused absences shall render the same hours to
compensate for the absences or tardiness. However,
unexcused absences and tardiness shall be penalized twice
(2x) the number of hours of absence or tardiness.

5.) The course uniform shall be worn on Mondays and


Tuesdays, or for at least two days in a week, as
preferred by Cooperating Agency unless the Cooperating
Agency requires another set on uniform.

6.)Practicum students are expected to fulfill the Code of


Ethics of SLU and their respective Cooperating Agency.

7.) Extension beyond the required practicum hours shall


be the responsibility of the Cooperating Agency and the
student only.

8.) Complaints regarding the practicum student by the


Cooperating Agency must be coursed through the Adviser or
the Department Head, through call or text, but with
accompanying complaint letter duly sealed in an envelope
to be submitted by the student to the Adviser or
Department Head.

9.) Complaints of the practicum student to their


respective Cooperating Agency must be coursed through the
Adviser or the Department Head, through call or text, but
with accompanying complaint letter specifying the grounds
for complaints. The faculty adviser or the Department
Head shall coordinate to the Cooperating Agency about the
complaint, and if grounds are substantial, the Adviser or
the Department Head can transfer the practicum student to
another Cooperating Agency.

10.)The student shall request a Certificate of Completion


from the Cooperating Agency and shall submit the same
with the corresponding photocopy to the Adviser, as one
of the requirements for completion of the practicum
course.

11.) Cooperating agency visits will be conducted at least


three times a semester by the Adviser or as per agreement
between the Cooperating Agency and Adviser and can be
arranged as inter-departmental or intra-departmental
visits.

12.) The adviser shall meet the class during the first
week of classes, and at least once every grading period
for the purpose of collecting the requirements. Absence
from Cooperating Agency is excused during these periods.

13.) The adviser shall make a list of the Cooperating


Agencies where all his/her respective interns are having
their OJT and all will be consolidated to serve as a
basis of identifying the number of Cooperating Agencies.

14.) The adviser shall conduct random visit at the


respective workplace of the practicum students. He/she
shall interview the supervisor of the practicum student
for possible feedback or improvement for the practicum
student.

15.) The adviser may visit the Cooperating Agency when


requested by the Cooperating Agency to discuss matters
relation to the on-the-job training of the student. The
adviser may get feedback from the Cooperating Agency and
if the feedback matters to the department’s practicum
program, he/she shall report to the Department Head and
the Department Head communicates the feedback to the
faculty members.
