AFFIDAVIT OF LOSS

I, LAILANIE B. MAGARANG, of legal age, Filipino, single and a

resident of 42 Bato Ali St., Moncado Colony, Marawi City, Lanao del
Sur, under oath do hereby depose and say:

1. That I used to be a holder of a PRC ID with ID No. 1303701

issued to me by the Professional Regulation Commission
(PRC)-Cagayan De Oro City;

2. That when we fled Marawi City because of the siege, I

inadvertently lost/misplaced said PRC ID.

3. That by reason of such circumstance and the fact that the

said card is already beyond recovery, the same is considered
lost for all intent and purposes;

4. That I execute this affidavit to attest to the truthfulness of the

foregoing statement and for whatever worth this may serve
best.
