 INSTRUCTIONS

GENERAL

1. All Pakistani citizens are eligible for membership.

2. All attached documents should be duly attested by Oath Commissioner/Town Nazim/Magistrate

Class-I, Gazetted Officer (Garde-17 & above).

3. Original CNIC MUST be valid & in possession while appearing before SANGAR Designated
Officer.

4. This Membership is for life and is renewable after every 03 years.

5. Applicant is required to appear personally before the SANGAR designated officer for
signatures, digital photograph and digital finger printing.

6. Biometrics of the applicant will be done at the time of form submission.

COMPLETION OF FORM

1. In case of more than one marriage, a detail of all wives are required.

2. All attached documents must be readable.

3. No overwriting/cutting/erasing is acceptable.

4. Incomplete forms will not be accepted.

5. No entry is to be left blank.

6. All columns must be filled in BLOCK LETTERS.

DOCUMENTS TO BE ATTACHED

1. Pay Order/Bank Draft against Registration fee payable to SANGAR HOUSING PROJECT,
GWADAR, Karachi of Rs.10,000/- (RUPEES TEN THOUSAND ONLY). Please attach a
photo copy of the same.

2. Two latest passport size photographs duly attested (One on front & one on back).

3. Two copies of valid CNIC duly attested.

4. Attested photocopy of NADRA Form 'B' for family members under 18 years of age.

5. Copy of valid Passport / CNIC of children over/under 18 years for persons having Overseas
Pakistani Card.

6. Affidavit on Rs. 100/- Non Judicial stamp paper duly attested by Oath Commissioner
(Specimen enclosed).

7. 1x passport size photograph of each living family member & photo copy of CNIC duly
attested (Over 18 years of age).

8. Members having Sangar plot files are requested to attach the copies of each file with the
form.
