Dear Sir/Madam:

Pursuant to Paragraph 6 of Section 14, Republic Act No. 7166, the undersigned
[RED/PES/EO] is hereby reminding you to comply with your obligation to file your
Statement of Contributions and Expenditures (SOCE) and all its required attachments
with this office on or before Wednesday, 08 June 2016. This deadline shall not be
extended. YOU ARE REQUIRED TO FILE YOUR SOCE even if: you were not
elected, your campaign was self-funded, you incurred no expenditures, or you did
not pursue your campaign as long as you did not withdraw your candidacy before
10 December 2015. Failure to file the said Statement within the period provided shall
subject you to the penalties provided for under Section 14 of Republic Act No. 7166.

Kindly refer to COMELEC Resolution No. 9991 for the Omnibus Rules on Campaign
Finance for instructions on how to file the disclosure statements and reports. Soft or
electronic copies of the prescribed forms can be downloaded from the COMELEC
website (www.comelec.gov.ph). Please note that:
 All entries must now be encoded – handwritten entries are no longer
accepted and shall be considered as NOT FILED if NOT ENCODED;
 Incomplete reports and statements that contain blank information, that do not use
the new prescribed forms, and do not have the necessary supporting documents
such as photocopy of contribution and expense receipts – shall be considered as
NOT FILED.
 All candidate-related forms (Forms SCE, SCR, SOE, SLE, SUO, SER, etc.) must
be personally signed by the candidate him/herself and duly executed under
oath or notarized – if not, they shall be deemed as NOT FILED.
 All party submissions must be signed by the party treasurer.
 Soft copies of the accomplished forms must accompany the final signed,
dated and notarized submissions, or else they will be deemed INCOMPLETE
and NOT FILED.

You are responsible for making sure that all your submissions are complete, signed,
dated and notarized.
