 Introduction.
Doctors often have trouble diagnosing the disease because of a lack of
patient information and laboratory tests that the patient may have
forgotten or lost because they are written on paper or lose the CD
where the patient's information is stored, or because of his transfer
from one hospital to another, or maybe the patient is unconscious to
give information,Which makes it difficult for the doctor to diagnose the
patient's condition accurately.

So we need certain systems to save patient information, laboratory


tests, health history, medicines and other information needed for
diagnosis and treatment, such as electronic health records (using
database in healthcare).

What is a patient database? And what is electronic health records?


Patient data is data relating to a single patient, such as his/her
diagnosis, name, age, earlier medical history etc.

This data is typically based on a single patient-health care worker


interaction. ... Patient based data is important when you want to track
longitudinally the progress of a patient over time. We can apply all this
using electronic health records

 How to deal with this problems:

to avoid these problems , we can use database in healthcare and EHRs to get
all patient information , by creating single account for each patient contains
all patient information since first admission to hospital in clients life.
That in future or next time can back to these information without any losing
of information, and can use the account in any other health organizations ,
we can say it's like a Facebook account we can access to in any time or any
place with username and password.

 For example :
A 75 years old male patient was admitted to the hospital complaining of
cough , chest pain , and difficulty in breathing , he was admitted before one
year ago to another hospital , the patient is old and e lost his ECG and
radiology results and lab tests and didn’t remember anything about his case
, so the physician need to know his past health history , lab tests ,ECG , and
ect , to diagnosis him in accurate . That’s need to contact with the last
hospital he was admitted to , and this may take time!
So we need to apply the idea for these real problems

 How the system work ?

The system is going to work by linking codes of) patient information ,past
medical history ,laboratory values ,Assessment findings, Radiology result,
medication allergies, etc.).
These system keep all clients information and never lose it , and can add
new information by physician when patient visit any Health care institution
,and can go back to the old patient's information and take advantage of
them ,also patients can access to their accounts to see the results and
medications ,etc. The system easy to use for anyone can deal with .
In other words, the system collects results, information and analyzes and
saves them in the patient's account and uses them in any other health
sector for easy access to patient information. Maybe can access to the
system or the account in another country if patient transferred out.

 Why we need databases in healthcare (the benefits of the system)?

Databases have significant benefits to help the health sector work, such as :
1. Increased accuracy when doctors diagnose diseases.
2. Minimize the occurrence of errors in the diagnosis.
3. Reducing costs such as image tests performed by the patient are not
repeated.
4. The ability of doctors to see the patient's old information such as
tests, treatments, allergies of medicines, genetic diseases.
5. Save time on patient and doctor; instead of wasting time finding and
collecting patient information.
6. In the event that the patient cannot give the doctor information, the
doctor can complete his work without such as the patient is in a
coma.
7. Keep patient information from loss.
8. It is possible to insert information about the doctor's condition into
the system.
