Describe the essential elements of a successful health and safety management system.

hazards, assessing risks and establishing standards against which the organisation can
measure performance

Why is it important to have an occupational health and safety policy?


A written policy statement is a preferred method of communicating the organisation’s goals
and undertakings to workers. It provides evidence of commitment by management to increase
health and safety performance for all workers. The commitment from management develops
by identifying roles and responsibilities for occupational health and safety issues and
establishes parameters/guidelines for site-specific procedures. Therefore, communication of a
health and safety policy may be carried out by integration into induction material, and
incorporation into duty/role statements, procedures manuals and job descriptions

The potential for work accidents to cause emergencies has increased. Why?
It has increased due to the complexity of processes, the proliferation of industrial and
agricultural chemicals, and the often close proximity of residential areas to industrial
activities.

Describe the elements of a basic emergency preparedness plan.


The basic plan will usually include: a chain of command, an alarm system, medical treatment
plans, a communications system, shutdown and evacuation procedures, and auxiliary power
systems.
 All possible emergencies, consequences, required actions, written procedures, and the
resources available.
 Detailed lists of personnel including their home telephone numbers, their duties and
responsibilities.
 Floor plans.
 Large scale maps showing evacuation routes and service conduits (such as gas and
water lines).

By way of example, justify that different emergency response procedures have to be


applied for different emergency situations.
Specific emergency procedure is required for specific emergency events; as example:

Fire
Leave the building via the nearest safe route.
Close the doors as you go – do not lock them
Obey all directions from wardens.
Move calmly to the assembly point or other advised area

Bomb Threat
If you are told by emergency responders to evacuate the building:
Evacuate the building via alternate exits, leaving doors and windows open.

Use stairs only; do not use elevators.
Take personal belongings with you, noting any suspicious parcels in your area as you leave.
Move to assembly point maintaining a clear distance from parked vehicles.
Move well away from the building and follow instructions of emergency responders.

How severe must an incident be before it is investigated?


Each incident that resulted in or could reasonably have resulted in an injury and/or fatality
must be investigated.

Give FOUR (4) reasons why there should be a system for the internal reporting of
accidents in an organisation.
• The compilation of accident statistics and identifying trends
• To meet the requirements of the Reporting of Injuries, Diseases and Dangerous
Occurrences Regulations 1995
• Investigations may be carried out to prevent future occurrences
• Use in civil claims or to satisfy insurance requirements
• Help in the identification and reduction of loss
• Inform the review of risk assessments.

Outline the factors that could prevent accidents from being reported at work by

• Preservation of the company’s or departments safety record (particularly where
incentive schemes are in place)

At a minimum, what information should an incident investigation report cover?


An incident investigation report should cover the date of the incident, the investigation start
date, a description of the incident, a list of contributing factors, root cause assessments,
recommendations, follow- ups, and resulting actions.
