EXECUTIVE SUMMARY

DWCV-COLLEGE STUDENTS’ DISCIPLINARY RECORDS MONITORING

SYSTEM.Ysvin L. Racsa, Arlyn P. Diolola and April V. Rapadas.Bachelor
of Science in Information Technology.Divine Word College of Vigan
(Vigan City, Philippines), 2016.76 Pages. Adviser: Julius Cezar Q.
Palomares, BSCS.

This project aimed to develop the DWCV – College Students’

Disciplinary Records Monitoring System for the Students’ Affairs Office of
the Divine Word College of Vigan.
The DWCV – College Students’ Disciplinary Records Monitoring
System was developed to help the office of Students’ Affairs to manage
information about the violations and sanctions of the students who
committed offenses.
The development of the system employed the four (4) phases of the
Spiral Model: Identification, Design, Construct or Build, and Evaluation. In
the Identification phase, the proponents conducted interviews with the

Head of the Students Affairs Office of the Divine Word College of Vigan to
determine the different problems encountered in the said office. In the
Design phase, the proponents described in details the graphical user
interface of the system. The proponents created flowcharts to illustrate the
flow of the system. In the Construct and Build phase, the development and
improvement of the DWCV – College Students’ Disciplinary Records
Monitoring System took place where the proponents developed the entire
system with the different functions of each forms. In the Evaluation phase,
the proponents ensured that all errors were fixed and the system met its
necessary specification.

ix

In the beta testing, the proponents used an evaluation tool with the
rating of highest of five (5) and lowest of one (1). The beta-testers gave
comments and suggestions about the developed system including its
security.
It was then recommended that the Students Affairs Office should
employ the system to help in monitoring violations, and to help in
constructing violation reports in a secured process using the new instrument
from the modern technology.
