OWNER-CONTRACTOR AGREEMENT

(ADDENDUM/AMMENDMENT)

KNOWN ALL MEN BY THESE PRESENTS:

This ADDENDUM, made and entered into this [Insert date here] day of [Insert month here] at
[Insert place here], Philippines, by and between

THE GIDEONS INTERNATIONAL IN THE PHILIPPINES, INC., an association duly established under
the laws of the Republic of the Philippines, represented herein by its National President, Mr. Rovic T.
Jereza, with office address at Panay Avenue, Brgy. Paligsahan, Quezon City, hereinafter referred to as
the OWNER;

-and-

B.S. & S. BUILDERS, INC., a domestic corporation, organized and established under laws of the
Republic of the Philippines, represented herein by its President/COO, Engr. Jonnel F. Balajadia, with
office address at Rockwell Drive, Brgy. Poblacion, Makati City, hereinafter referred to as the
CONTRACTOR:

WITNESSETH

WHEREAS, the CONTRACTOR has an existing contract with OWNER as evidenced by the Owner-
Contractor Agreement dated May 24, 2017 hereto attached as ANNEX A and hereinafter referred to as
the FIRST CONTRACT;

WHEREAS, in addition to such, the CONTRACTOR was awarded ADDITIONAL WORKS and the
parties desire to extend the term of the current contract for additional time to February 05, 2018,

WHEREAS, the ADDITIONAL WORKS, is as follows and hereto attached as ANNEX B:

Additional Works No. 01: PHP 497, 013.38
Additional Works No. 02: PHP 1,983,885.44

NOW, THEREFORE, the parties agree to modify the following parts of the agreement: Section 2
Time of Completion and Liquidated Damages, as set forth below.
 TERMS AND CONDITIONS

NOW THEREFORE, for and in consideration of the mutual covenants herein contained, OWNER
and CONTRACTOR agree as follows:

OWNER agrees to grant an extension to the date of completion as requested by the

CONTRACTOR in total of 35 DAYS as evaluated by MCD Project Managers, Inc.;

A. Section 2 of the Agreement with the Contractor, dated May 23, 2017 is hereby amended to
extend the contract termination date from December 31, 2017 to the following:

1. This Agreement shall commence on January 01, 2018 and shall terminate on February 05,
2018;
2. Except as otherwise provided herein all terms and conditions of the agreement shall remain
in full force and effect;
3. Updated Certificate of Insurance(s) attached hereto.
