2. That on May 21, 2018, said sim card inserted my cellphone was lost while I was at
the vicinity of Balas, Mexico, Pampanga and the same could no longer be found;

3. That diligent effort to locate said sim card proved futile;

4. I am executing this affidavit to attest the veracity of the foregoing facts

and circumstances and for whatever legal intent and purposes this may serve.

Copies of my Barangay Identification Card and Solo Parent Identification Card are
hereto attached as my proof of identification.

10. That I entrusted my cellphone which contained said sim card to my daughter;

11. That sometime in April 2018, said cellphone was snatched while my daughter was
aboard a passenger bus going to Manila and the same could no longer be found;

12. That diligent effort to locate said sim card proved futile;

13. I am executing this affidavit to attest the veracity of the foregoing facts
and circumstances and for whatever legal intent and purposes this may serve.
