Performance Evaluation is the appraisal of the accomplishment of each trainee. This will provide the occasion to
point out how well the trainee is doing his/her job and how he/she can improve it. This will also provide the
trainee the opportunity to set the job objectives in managing assigned tasks and assess achievement of
objectives.

3 = Satisfactory
2 = Needs Improvement

reasoning and good common sense.
7. ORGANIZATION

8. INITIATIVE
The extent to which the trainee is a self-starter and is 5 4 3 2 1
continuously motivated in accomplishing his/her job.
9. INDUSTRY
The extent to which the trainee may be described as a

committed worker and the amount of concentration and effort
extended in the performance of his/her job.
