: Mr. Ashok Kumar, 61- years old male is a case of compensated cryptogenic CLD
[Biopsy suggestive of Cirrhosis with Fibrosis], underwent UGIE [2014] suggestive
of PMG, now came in Emergency with complaints of easy fatiguability, abnormal
distention since 10-15 days. No history of GI bleed. Now admitted for further
evaluation and management.

: Patient was admitted under Gastroenterology with above mentioned complaints
for which relevant investigation were done. During hosptial stay, 2 unit of PRBC
was transfused in view of low hemoglobin [5.9mg/dL]. Patient was given Injection
Encicarb in view of reports suggstive of Iron deficiency anemia.Orthopedic opinian
taken for annular tear and disc prolapse,adviced physiotherapy and planned for
root block once medically fit.During hospital stay patient was managed with IV
antibiotics and other supportive measures. UGIE was done on 24/05/2018 and
source of blood loss was not found hence planned for Colonoscopy and
haematolgy review. But on patient persistent request for discharge. Now patient
is being discharged on request with given advice for follow up in OPD.
