MINUTES OF THE FACULTY MEETING HELD ON SEPTEMBER 29, 2017 AT THE


LABORATORY ROOM
Meeting Number 14

1 I. The meeting started with the dissemination of the school head, Jane Marie M. Ngolab of the
2 needed addition for the upcoming mid-service review and evaluation program. She emphasized that
3 the Schools Division Superintendent encouraged schools to infuse necessary things that are
4 needed in the professional development of teachers.

5 The school head also added that the Assistant Schools Division Superintendent together with the
6 different supervisors will be formulating a unified IPCRF. The faculty was informed that the special
7 task might be discarded.

8 II. The school head asked Mr. Emilio P. Vinoya to distribute request forms to teachers. Teachers will
9 list down the things they needed to be listed for the annual procurement plan.
10 Deadline will be on Monday ( October 2, 2017). Estimated prices must be included.
11 Equipment can be requested as long as it will not go beyond 10,000 pesos.

12 The school’s MOOE for 2018 is 995,000 pesos.

13 The school head called for the faculty’s active participation in the process of liquidating the school’s
14 MOOE. The school was almost not given or downloaded with a budget. This is because as of
15 August, 80% of the budget has only been liquidated.

16 The deadline of the liquidation is on the 5th of every month but the division’s deadline is every end of
17 the month to meet their IPCRF’s standard.

18 The school head proceeded with the discussion of the medical report as presented by Dr. Zosa and
19 Dr. Orpiano. Quezon Hill National High School ranked number one in the obese category.

20 It has been advised that teachers should avoid sedentary lifestyle to avoid illness.

21 In connection to this, the MAPEH department was assigned to conduct an exercise program for
22 teachers every Wednesday.

23 III. The meeting continued with the reminders from the administration office headed by Sir Bobby
24 Gonzales. The school head emphasized the following:
25 - Teachers can check the DepEd Pines Website (or the internet) for the requirements on ERF.
26 - Leaves must be processed two weeks to 1 month.
27 - Sick leave must be submitted upon return to duty.
28 - When absent, inform the school head and the year level coordinator.
29 - If the absentee did not inform the school, Form 6 will not be signed.
30 - Teachers must fill up all necessary information and must follow the protocol.

31 The Annual Procurement Plan (APP) will be revised by October.

1 IV. UPCOMING ACTIVITIES: Ma’am Lea Ticman and Ma’am Angeline Minos together with their
2 students will be attending the Division Science Festival on October 6-7.

3 On the other hand, all math teachers will be attending the Division Math festival on the same date.

4 West Baguio and Baguio Central District will conduct a journalism seminar on October 7 and 14.

5 Ma’am Lea Ticman and Ma’am Gemalyn Awingan will attend the Inclusive Education Conference on
6 October 12-13 at Eurotel.

7 Grade 10 students will be attending the Career Guidance orientation at the PFVR on October 3,
8 2017 from 7:30-12:00. Advisers must send a letter and a waiver to the students. Students will go
9 straight at the PFVR.

10 The school head will be attending the Education Congress from October 4 to 5, 2017.

11 V. NAT. Grade 7 students will be having their National Achievement Test (NAT).

12 Teachers must be guided by the following best practices of other schools/Division:


13 Have an area on the board to place the following:

15 Division Code
16 School ID
17 Grade 8 teachers will go out as proctors and they are:
18 a. Landy Minos
19 b. Gey-steve Ana-ao
20 c. Ramil Estioko
21 d. Mark Signabon
22 e. Angeline Minos
23 f. Rex Rulloda
24 g. Mary Jane Ganancila
25 h. Gemalyn Awingan

26 NAT takers will be using the grade 9 and 10 building. Grade 9 students will therefore transfer to the
27 grade 7 classrooms.

28 The meeting continued with the orientation of the guidelines on Test Administration of NAT 2017.

29 Ma’am Rufina Isiguen must coordinate the NAT dry run of grade 7.

30 Last period teachers are requested to facilitate the fixing of the classrooms to be testing room ready.
31 Chairs must not be piled up too high.

32 In the exam paper, include LRN aside from the student’s name. This will help students memorize
33 their number.

34 Advisers must make two School Form 2. One that has the list correlated to the LIS and the other is
35 correlated to the class record. Everyone must be specific in providing data because it can be used
36 for research.

37 Advisers must be mindful that there is no more unenroll facility and so everyone is responsible in
38 the drop out of students.

39 VI. OTHER ACTIVITIES. The school head announced other schedules:

8 The meeting was adjourned at 5:32 PM.
