I. PRE-INTERNSHIP

A. Requirements for Admission


1. Has taken at least 80% of the major subjects or 60 units.

B. General Procedures
1. Student will appear on the pre-evaluation and evaluation dates.
a. Grades in the preceding semester should be secured.

b. INCs should have been complied.

c. Pay the internship fee

2. If qualified, student will attend the orientation.

a. Pay the necessary fees (OJT Manual, operational expenses and uniform).

b. Wear your business attire. (Long sleeves and slacks with leather shoes
for men. Dark coat and pencil with dark footwear for ladies.)

3. The group will elect the set of officers who will help the Faculty Intern
Coordinator (FIC) to consolidate the documents and facilitate all internship
activities.

4. Submit the following to the FIC for scrutiny and signature:

a. Received application letter

b. Resume’

c. Waiver (student and school’s copies)

d. Signed MOA/MOU

5. Interns will secure an approval from the head of office they wish to become
interns.

6. Agreement between the interns and head of office assigned will be


determined and submit the certificate of acceptance to the FIC.

7. The intern will follow the enrolment procedures of the university. Students
will only start their internship after they have been validated. Put the
following in a green long folder together with the aforementioned. Submit to
the Faculty Intern Coordinator.

a. Photocopy of enrolment receipt

b. Photocopy of study load

c. Photocopy of Official Receipt of enrolment

II. POST-INTERNSHIP

A. General Procedures

1. At the end of the internship, each intern will consolidate the requirements
and put them inside a long green expanded folder following the order:

a. Signed DTR with complete data

b. Photocopy of logbook indicating signature of attending supervisor


authenticating the document

c. Weekly Journal

d. Narrative Report (follow specifications/format)

e. Certificate of Completion

f. Copy of received “thank you letter” to the assigned office head

g. Agency evaluation/grade sheet should be sealed in a white envelope.


Sealed part should be signed by the agency supervisor.

All documents should be passed at most 5 school days after the last day of the
internship.

2. The FIC shall call for a Colloquium. Each student will present a graded 10-
minute powerpoint/video presentation regarding his experiences. The
presentation may include, but not limited to:

a. Brief Office Profile. Where is it located? To whom does the intern directly
report to? What is main mandate of the agency and the unit/section
he/she is assigned to?

b. Duties and functions assigned to the intern.

c. Challenges/Difficulties encountered and the measures done to address


them.

d. Learning experiences

e. Tasks in other significant events

Note: Report should be supported with pictures and captions

3. The officers will help the FIC to bind all outputs.

4. The interns are graded according to Manner, Method and Matter.

5. The overall criteria in grading the interns includes work performance to be


rated by the supervisor, attendance to orientation and colloquium, presentation
of intern experiences and prompt submission of, accuracy of, and substance of
the documents sought.

REQUIREMENTS

Narrative Report Guidelines and Format

Report lay-out should typically be as follows:

1. Report cover ( a sample is provided, see page 35)


2. Contents with pages
3. Content of Report with appropriate sub-headings
4. Work cited section
5. Appendices section (pictures, letters received/approved, awards, citation,
etc)

Report content should cover information such as:

I. Introduction
- An introductory section providing an overview of the content of the report.

II. Office
- The name of the business/organization that you interned with.
- Identify the type of Business or agency and the specific agency. Provide some
history and context of the organization.

III. Office Assignment


- What division or unit were you assigned to? (If relevant)
- Identify the people and their titles in the division or unit you worked in.

IV. Goals and Learning Objectives

- What goal(s) did you accomplish during your internship?


- List objectives to meet that goal. These should be the same as those agreed
upon between you and the supervisor at the start of the placement.

V. Duties and Responsibilities


- Describe some of the duties and responsibilities given to you during this
internship.

VI. Challenges
- What challenges did you encounter during your internship?
- How did you resolve these challenges?

VII. Learning Experiences


- Discuss the relationship with your supervisor(s) and co-workers.
- Which specific theories/concepts/principles from which specific classes did you
apply or find useful as you developed and completed your internship?
Provide citations as appropriate.
- What lessons from this practical experience would have been difficult to learn
merely from a classroom?
- What were the best and worst experiences you had during your internship?

VIII. Conclusion
- Provide a succinct conclusion summarizing the key points covered in your
report. Review the following information, and return to it when you start writing
your report.

Notes: 1. Support report with pictures with captions.


2. Report must be encoded. (Specifications: at most 10-page report; long
bond paper; font 12, times new roman; 1 inch- margin all sides, 1.5-
spaced)
3. Pagination should be placed at the upper right. Page 1 starts after the
contents.
