Special Power of Attorney

Know All Men by These Presents:

That I, _________________ Filipino citizen, of legal age, married, with

residence and postal address at _____________________Philippines, do hereby
name, constitute, and appoint and by these presents do hereby name,
constitute and appoint son, ___________________ likewise of legal age, married
and a resident of the same address, to be my own and lawful Attorney-in-Fact,
for me and in my name, place and stead, to do and perform any and all of the
foregoing acts and deeds to wit:

1. To prepare, file, submit, pay and process all transactions related to the
application of my business permit;

2. To prepare, file, submit, pay and process all transactions related to the
application to secure Government for my business, particularly for SSS,
Philhealth, Pag-ibig and Tax Identification numbers;

3. To request, make, sign, execute and deliver all the necessary

documents in relation to my application for business permit and government
mandated numbers;

4. to duly receive in my behalf the legal notices and documents

regarding my business permit, SSS, Philhealth, Pag-ibig and TIN application;

5. To do and perform any and all acts necessary for the faithful execution
of the foregoing acts and deeds.

HEREBY GIVING AND GRANTING, unto my said ATTORNEY-IN-FACT full

power and authority to do and perform any and every act and thing
whatsoever requisite or necessary or proper to be done in and about the
premises as fully to all intents and purposes as I might or could lawfully do if
personally present, with power of substitution and revocation, and hereby
ratifying and confirming that my said attorney-in-fact shall lawfully do or cause
to do under and by virtue of these presents.
