Case #1
• PH is a 57 year old male patient at dental office for a routine checkup. His vitals at the
appointment are BP: 142/85, HR: 67. His PMH: hypertension (moderately controlled),
asthma (he mentions stress is a trigger for his asthma) is recently uncontrolled. He
complains of having more asthma exacerbations since he started new antihypertensive
medication. He also admits to smoking ½ pack/day usually the 1​st​ cigarette within 45
mins of awaking. He is nervous regarding the dental treatment today.
• You are expecting your patient to have severe inflammatory pain after having
orthognathic surgery today. This pain lasts for 2-3 days.
• Patient has concerns about being tolerant to analgesic effects of pain killers.
• Medication list:
• Metoprolol tablet 100 mg PO bid
• Albuterol inhalation spray every 6 hour prn…
• Centrum multivitamin 1 tablet PO daily
• Losartan 50 mg PO daily
• Loratidine 10 mg PO daily prn…

Metoprolol - Beta blocker


It can treat high blood pressure, chest pain (angina), and heart failure. This may lower the risk of death after a heart attack.

Albuterol - Bronchodilator
It can treat or prevent bronchospasm.

Losartan - Antihypertensive drug


It can treat high blood pressure. It can reduce the risk of stroke in patients with high blood pressure and an enlarged heart. It can
also treat kidney disease in patients with diabetes.

Loratidine - Antihistamine
It can treat allergy symptoms and hives.

Case #2
• Your 48 year old male patient complains of chronic dry cough which bothers him very
much.
• Patient also mentioned his physician prescribed him an anti-hypertensive med to possibly
prevent his nephrolithiasis recurrence, which happened three times in the last 4 years.
• You are going to extract his right and left maxillary 3​rd​ molar teeth, which are already
erupted.
• Medication list:
• Lisinopril 20 mg PO daily
• Hydrochlorothiazide 25 mg tablet
• Acetaminophen 325 mg caplet
• Ibuprofen 200 mg gel tab
• Diltiazem ER 240 mg PO daily
• Dabigatran 150 mg PO bid

Lisinopril - ACE inhibitor


It can treat high blood pressure and heart failure. It can also reduce the risk of death after a heart attack.

Hydrochlorothiazide - Diuretic
It can treat high blood pressure and fluid retention (edema).

Diltiazem - Calcium channel blocker and antihypertensive drug


It can treat high blood pressure and chest pain (angina).

Dabigatran - Blood thinners


It can treat and prevent blood clots, reducing the risk of stroke.

Case #3
• You have seen your 49 year old female patient for her routine dental appointment today
and noticed her recent gingival hyperplasia. She also complained of bleeding from her
gums every night when she brushes her teeth.
• Patient has a history of hypertension and epilepsy, both controlled with medications in
the past. Patient is nervous regarding her dental treatment today.
• She smokes cigarette with moderate nicotine dependence.
• Patient’s BP is 150/92 today
• Medication list:
• Valsartan 40 mg PO daily
• Aspirin 81 mg PO daily
• Phenytoin 300 mg PO tid
• Nifedipine ER 60 mg PO tablet daily
• Alendronate 70 mg po weekly
• Pseudoephedrine 60 mg PO tid (started 2 days ago)

Valsartan - Antihypertensive drug


It can treat high blood pressure and heart failure. It can also lower the risk of death after a heart attack.

Phenytoin - Anticonvulsant
It can treat and prevent seizures.

Nifedipine - Calcium channel blocker and antihypertensive drug


It can treat high blood pressure and chest pain (angina).

Alendronate - Bone health


It can treat or prevent osteoporosis. It can also treat Paget's disease of the bone.

Pseudoephedrine - Decongestant
It can treat stuffy nose and sinuses.

Case #4
• You have a 56 year old female patient for her dental appointment with the chief
complaint of pain and swelling in her lower jaw. After examination and looking at her
radiograph, you noticed a radiopaque mass with a sunburst pattern in her right
mandibular bone. After referring to a colleague at the oral maxillofacial surgery clinic
you decide to send her for a CT scan with and without contrast.
• Patient also complains dry mouth
• She also has heartburn and started OTC medications
• Medication profile:
• Aspirin 81 mg po daily: antiplatelet
• Valsartan 50 mg po daily: ARB
• Metformin 1000 mg po bid: diabetes medication
• Sitagliptin 100 mg po daily: diabetes medication
• Calcium carbonate 500 mg po bid: antacid
• Atorvastatin 40 mg po daily: co-HMG Co A reductase inhibitor: statin
• Vitamin D 1000 units po daily
• Omeprazole 20 mg po daily: PPI
• Duloxatine 30 mg po bid: SNRI

Valsartan - Antihypertensive drug


It can treat high blood pressure and heart failure. It can also lower the risk of death after a heart attack.
Metformin, Sitagliptin - Anti-Diabetic medication
It can treat type 2 diabetes.
Atorvastatin - Statin
It can treat high cholesterol and triglyceride levels. This may reduce the risk of angina, stroke, heart attack, and heart and blood vessel
problems.
Omeprazole - Proton-Pump inhibitor
It can treat heartburn, a damaged esophagus, stomach ulcers, and gastroesophageal reflux disease (GERD).
Duloxatine - Nerve pain medication and antidepressant
It can treat depression, anxiety, diabetic peripheral neuropathy, fibromyalgia, and chronic muscle or bone pain.

Case #5
• Your 36 year old female patient is at your clinic for the extraction of her painful and
abscessed tooth. During the examination, local cellulitis and swelling was noted. She has
no significant medical history except for asthma with frequent exacerbations in the past 2
months.
• Right after extraction she mentioned that she takes oral contraceptive pill
• Medication profile:
• Albuterol inhaler 2 puffs every 4 hours:SABA
• Salmeterol- fluticasone inhaler 1 puff twice daily: LABA
• Medrol dosepak (methylprednisolone): Steroid
• Montelukast 10mg po daily: LA

Albuterol - Bronchodilator

Salmeterol - Bronchodilator
It can prevent asthma attacks and exercise-induced bronchospasm. It can also treat COPD, including chronic bronchitis and
emphysema.

Methylprednisolone - Steroid
It can treat inflammation, severe allergies, flares of chronic illnesses, and many other medical problems. It can also decrease some
symptoms of cancer.

Montelukast - Anti-Inflammatory
It can treat allergies and prevent asthma attacks.

Case #6
• Your 59 yo male patient with a past medical history of MI and had one drug eluting stent
placement in his coronary arteries 3 months ago. His appointment today is for a right
maxillary canine and central simple extraction. These teeth are not restorable and very
painful.
• Medication list:
• Aspirin 325 mg po daily
• Clopidogrel 75 mg po daily
• Metoprolol 50 mg po bid
• Amlodipine 5 mg po daily

Clopidrogrel - Blood thinners


It can prevent stroke, heart attack, and other heart problems.

Metoprolol - Beta blocker


It can treat high blood pressure, chest pain (angina), and heart failure. This may lower the risk of death after a heart attack.

Amlodipine - Calcium channel blocker


It can treat high blood pressure and chest pain (angina).
