PETITION

COMES NOW THE PETITIONER, LYNDON EVANS P. MONDAY,

in his own behalf, and unto this Honorable Court, most respectfully
states that:

1. He is a Filipino, of legal age, single, and a resident of 613 Palm

Drive St., Buhangin, Davao City;

2. He was born on January 1, 1987 in Davao City, Philippines;

3. He has satisfactorily completed the required course in Law at Cor

Jesu College, on April 2016;

4. He passed the Bar Examinations given on November of 2016 and

admitted to the Philippine Bar on May 24, 2017 with Roll of
Attorney’s No. 67143;

5. He is a member in good standing of the Integrated Bar of the

Philippines, Davao Oriental Chapter;

6. He is a new applicant desiring to be commissioned as a Notary

Public for and in the City of Davao;

7. He is presently holding office at Unit 10, ATU Plaza, Gov. V.
Duterte Street, Davao City, Philippines, with telephone number
(082) 225-4168;

8. He possesses all qualifications and none of the disqualifications as

required by law;

9. He has no pending administrative case as certified by the Office

of the Bar Confidant of the Supreme Court

10. To support his Petition, the Petitioner submits the following

documents:

a. Photocopy of Certificate of Membership in the

Philippine Bar issued by the Bar Confidant, marked as
Annex “A”;

b. Clearance from the Office of the Bar Confidant,

marked as Annex “B”;

c. Certificate of Good Moral Character from IBP – Davao

City Chapter, marked as Annex “C”;

d. Impression of Notarial Seal marked as Annexes “D”,

“D-1”, and “D-2”;

e. Specimen signatures marked as Annexes “E”, “E-1”,

and “E-2”;

f. Photocopy of the Official Receipt from the

manufacturer of the Notarial Seal marked as Annex
“F”;

g. Photocopy of the Official Receipt of Professional Tax

Receipt No. 9904772 issued on January 5, 2018
marked as Annex “G”;

h. Three (3) passport sized photographs, as Annex “H”,

“H-1”, “H-2”;

 WHEREFORE, it is most respectfully prayed from this Honorable
Court that herein petitioner be commissioned as a Notary Public for and
in the City of Davao.
