INSTRUCTIONS

1. Please read the BCA test requirements 1 for imported coarse and fine aggregates before making a
submission for S1 Test.

2. Please complete all sections of this form (all fields are mandatory except stated otherwise).

3. The person signing this form must be one of the following officers of an incorporated entity or
partnership, namely Chief Executive Officer, Managing Director, Director, General Manager,
Partner or the Sole Proprietor of Sole proprietorship.

4. You are to submit BY HAND the following documents along with this form:-

(a) Test reports (indicating river / quarry / mining site and product type) indicating the results
based on tests conducted on different parts of the same sample in accordance to the tests
required by BCA as listed in the BCA test requirements for imported river / land sand, coarse
and fine aggregates,
(b) A copy of letter from the owner or authorised operator of the quarry / mining site that the test
reports are conducted on sample from the source,
(c) A copy of letter of authorization issued by the owner or authorised operator of the River /
Quarry / Mining Site (for authorizing the importer to obtain a supply of and to import the
sand/aggregates from that river / quarry / mining site),
(d) Detailed map showing the exact location of the river / quarry / mining site within the country
of origin,
(e) Three plastic containers (each container of sample weighs about 150g) clearly labelled with
the name of importer, name of river / quarry / mining site, type of material / product and date
of submission,
(f) The latest copy of business profile* obtained from the Accounting and Corporate Regulatory
Authority (ACRA) (*date of request of business profile should not be more than one month
from the date this application), and
(g) Geologist report of the quarry / mining site, if required by the BCA.

SECTION C : EVALUATION INFORMATION

Any test report submitted to BCA must be valid at the point of submission based on the following conditions:
(a) The Petrographic Examination and Mortar Bar Method test reports must not be more than 1 year old and must be conducted on
the same aggregate sample in the same laboratory,
(b) The Sulphate content test result must not be more than 3 months old,
(c) The Chloride content test result must not be more than 1 week old.
