To Whom It May Concern:

This has reference to our company’s predicament regarding the billing statement that your
company is supposed to send once every month to our company which unfortunately, your company
appears to neglect doing the same.

May we inform you that for the past seven (7) months, since August 2013 up to the present,
instead of receiving a monthly billing statement from PLDT, we ask for a billing statement from your
company through a customer service representative. However, it seems that you are neglecting to make
a prompt action on our concern. In fact, the billing statement after our request has been made through a
customer service representative takes a week or even more to be sent through fax.

Relative thereto, may we request your good office to please send us a monthly billing statement
(hard copy) to our billing address at _____________________ for we cannot afford to do a monthly
request through your CSR just to have a billing statement.

As your valued customer, we believe and trust that PLDT maintains the company’s values of
accountability, integrity, fairness and most of all, transparency.

Hoping for your favourable and prompt action on the above matter.
