AFFIDAVIT

I, Sharla Castillo, Filipino, of legal age, single, and a resident of , after having
been duly sworn in accordance with law, hereby depose and say:

1. That I am a holder of an Pagibig ID;

2. That I used to put said Pagibig ID together with other personal documents in
my wallet;

3. That sometime on May 2018, when I tried to look for the wallet in the place
where I usually put it, I could not find it;

4. That I have exerted all the efforts needed to find but to no avail;

5. That my SSS ID is now lost and beyond recovery.

6. That I am executing this affidavit to attest to the truth of the foregoing facts and
to request for a replacement of my SSS ID.
