RE: Performance Assessment (Warning Letter)

This warning letter serves as written notice to Engr. Jomar Lagmay, in

accordance with his performance duty being the Engineer-in- Charge
under the 12:00 midnight to 9:00 in the morning shift schedule (third
shift).

This written warning is based on your unsatisfactory performance for

consecutive shifting schedules which we have personally monitored.
We have observed that you are nowhere to be seen within the office of
the Engineers neither within the premises of the “supposed” to be
checked from time to time, most especially in your designated post
—“Entrance” of The Biopolis during the peak time. We also have
observed that for a quite two to three (2-3) hours stay in the
Engineer’s quarter which we would like to hear your explanation.

Effective immediately upon the receipt of this letter, you are expected
that within twenty-four (24) hours to make a written explanation
regarding the aforementioned unsatisfactory behavior during your
shift.

Failure to demonstrate immediate Notice to Explain may result to a

disciplinary action.
