AMICABLE SETTLEMENT

THIS SETTLEMENT AGREEMENT is entered into between:

(1) MARIBETH E. BUSICO, of legal age, Filipino, single, a resident of UPPER

SICAYAB, DIPOLOG CITY, driver of a MITSUBISHI MIRAGE COLOR RED
bearing plate number ADO 8869 which is lawfully owned by her;

(2) REYNALDO M. SASUMAN, of legal age, Filipino, single, a resident of

JOSE DALMAN, ZAMBOANGA DEL NORTE, and Driver of PASSENGER
VEHICLE MOTORCAB YAMAHA COLOR BLACK bearing plate number JY
7341, registered under the name of EDURNE C. VIDAD represent by
Harvel C. Vidad.

WHEREAS, on June 3, 2018, a vehicular accident transpired in Lacaya St.

Corner Echavez St. Central Brgy. Dipolog City between a Mitsubishi Mirage
vehicle with plate number ADO 8869 driven by MARIBETH E. BUSICO, and a
PASSENGER Motorcab with plate number JY 7341 driven by REYNALDO M.
SASUMAN.

WHEREAS, both vehicles involved were seized and taken into legal custody
by the Philippine National Police of Dipolog City – Traffic Management in
accordance with their rules and procedures.

WHEREAS, the parties desire to enter into this Settlement Agreement to

provide for certain payments in full settlement and discharge of confiscated
vehicles, and in which are or might become the subject of a Complaint, upon
the terms and conditions set forth herein;

WHEREAS, the owner of the Passenger Motorcab offers to pay PHP 15, 000
as payment for the cost of repair of the Mitsubishi Mirage, PHP 7,000 to be
given as partial payment and remaining balance to be paid in full upon the
approval and release of the insurance;

FINALLY, the parties hereby authorize the Chief of Police of PNP Dipolog
City Station to release the Mitsubishi Mirage vehicle with plate number ADO
8869 and PASSENGER Motorcab with plate number JY 7341.

NOW THEREFORE, for and in consideration of the foregoing

premises, MARIBETH E. BUSICO, hereby agree to settle the dispute with
REYNALDO M. SASUMAN represent by Harvel C. Vidad and pay the cost
REPAIRS and DAMAGES on the Hyundai vehicle called for herein.

In case of breach, MARIBETH E. BUSICO reserve her right to institute a

formal complaint in proper court against the other parties who will be liable for
more charges which includes cost of litigation and damages.

IN WITNESS WHEREOF, we have hereunto affixed our hands this 5nd day
of June, 2018 at Dipolog City, Philippines.

001804, issued by Land Transportation Office of Dipolog City,, as a competent
evidence of identity; and affiant REYNALDO M. SASUMAN who exhibited to me
his Driver’s License No. J01-99-028556, issued by Land Transportation Office
of Dipolog City, as a competent evidence of identity. Both parties personally
known to me to be the same person executed this deed in their free act.

This instrument, consisting of two (2) pages, including the page, has
been signed on the left margin of each and every page thereof by the concerned
parties and their witnesses, and sealed with my notarial seal.
