The Anthem Parkside at M errill Ranch (APM R) Community Center is made available for use by Anthem Parkside residential owners
and tenants, provided financial commitments to APM R are current and users abide by the guidelines as stipulated in the Community’s
Covenants, Rules, Regulations, Restrictions and Guidelines. M embership privileges include the use of the Community Center facilities
and the privilege of registering for the programs and classes as coordinated by APM R staff. This and all policies are subject to change

Designees
If a membership is issued in joint names, the members shall jointly designate in writing to APM R one member to exercise the privileges
of the membership (Head of Household), subject to the right to extend such use privileges to other authorized users as provided below.
Such designations may be changed no more than once in any 12-month period, subject to approval of APM R and payment of such
change fees as APM R may establish from time to time.

Authorized Users
In addition to the member or its Designee, additional persons may be authorized to enjoy the use privileges of the membership as
follows:
M embership entitles the member, one other adult residing in the same household with the member (''Designated Adult"), and the
dependent children of each under the age of 23 who either reside with the member or attend college on a full-time basis, to use the
Facilities in accordance with the use privileges extended to members. A member may request that APM R also authorize a child of the
member or the Designated Adult who is 23 years of age or older and who permanently resides on the member's Lot, or a parent of the
member or the Designated Adult who permanently resides on the member's Lot, to enjoy such use privileges while residing on the
member's Lot; provided, APM R may charge the member a separate fee or additional dues for each such additional person 23 years of
age or older who is authorized to enjoy such use privileges.
Membership Details
1. Resident Memberships - Each membership includes the Head of Household and up to three other adults and their
dependent children under the age of 23 who either reside with the member or attend school on a full-time basis. A maximum
of six (6) memberships will be issued for dependent children. At the age of nine (9), dependant children will receive their
membership cards. A maximum of 10 memberships will be issued per residence. Children who are under the age of nine (9)
and already have their cards will be grandfathered in, and will not have to surrender their cards. Children must be fourteen
(14) years of age to enter the facility without adult supervision.
a. Additional adult memberships are available for purchase and will only be issued to adults who permanently reside
on the member's Lot.
Additional members will have to prove residency with a valid AZ Driver’s License or AZ Identification Card
(that shows the APM R address) and pay for membership.
Pre-pay $175/person for one year.
1-year membership required.
Additional memberships only sold in 1-year increment.
Fees must be pre-paid and are non-refundable and non-transferable.
Dependent children of the additional adult members are eligible for a membership, but those who are 7
years and older will have to pre-pay $175 for their 1-year membership and are subject to item b. Children
do not count towards the limit of two additional adult memberships.

c. A resident’s membership may be transferred to a family member who is temporarily residing on the member’s lot for
a fee of $50. There is no fee to transfer the membership back to the permanent resident.

2. Tenant Memberships
a. Landlord must sign the Tenant M embership form and provide a signed copy of the Lease.
b. A $50.00 administration fee, payable by cash, debit or credit card (no checks or American Express Cards) is required
at time of membership activation.
c. All members living at the property must be listed on the Lease.
d. Same rules apply as resident memberships regarding additional members and documentation for dependents.
e. Lease agreements must be a minimum of thirty (30) days.

3. Daily Guest Fees – M embers are allowed to bring guests to the Community Center for a guest fee.
a. There are no limits to the number of guests members may bring in.
b. Guests of residents are required to pay the daily guest fee of $3.00 per person. A 10-guest punch pass, which is
valid for one year, is available for $25.00. Guests 6 years of age and younger are free.
c. Guests must complete a Guest Form each time they enter the facility.
d. Residents must accompany their guest(s) to check in at front desk and remain in the facility while guests are present.
e. M embers who are age 14-17 may bring a guest who is at least 14 years old or older.
f. Child care providers or family members who are non-members and are at least 18 years of age, may accompany
resident members under the age of 14 with prior written consent of the adult resident member of the family. A
completed guest form constitutes written consent.
g. M embers who are at least 18 years old may bring a guest of any age.

4. Unaccompanied Guest Pass


a. Available for purchase - $60 for seven consecutive days
b. Up to 4 individuals may come in on that pass
c. Only 10 unaccompanied guest passes will be available to each lot per year.
d. Owners must provide (in writing) the names, ages, and contact information for all individuals on the pass.
e. Pass holders must agree to the same terms and conditions in the rules and regulations as any other membership
holder, including all age restrictions.

5. Age Restrictions
a. The Fitness Center and Pools are restricted to age 14 and older unless supervised by an adult (18+) or participating
in an Association sponsored children’s activity or program.
b. The lower level of the Community Center, except the pool, the Water Park and the East Lawn, is restricted to age 9
and older unless supervised by an adult (18+).
All fees must be pre-paid and are non-refundable, non-transferable and non-prorated.
Good Standing
A member, Designee, or authorized user shall cease to be in "good standing" upon the occurrence of any of the following:
(a) failure to pay to any APM R fees, dues, or other charges, or any installment thereof, on or before the due date;
(b) failure to accompany a guest if and when required while using the Facilities or allowing another person to use your I.D.;
(c) resignation or other termination of the membership by which the member, Designee, or authorized user was entitled to use the
Facilities (i.e. selling/leasing home);
(d) violation of these M embership Guidelines or the Rules, Regulations, CC&Rs of the Community;
(e) conviction of a felony or of any crime involving moral turpitude, or a determination by APM R that the person was convicted of
a felony or such a crime prior to APM R Management’s approval of his or her application and failed to disclose such conviction
prior to such approval; or
(f) commission of any act which APM R M anagement determines to be detrimental to or likely to endanger the welfare, safety,
harmony or good reputation of APM R, the Facilities, staff, or any member, Designee, or authorized user.
Management, Operation of APMR Facilities
APM R shall have exclusive authority and shall be responsible for the management and operation of the Facilities, which shall include,
without limitation, the following:
Staffing: APM R shall select, retain, supervise, direct, fix the compensation of, and discharge, in its sole discretion, all professionals and
other personnel, agents, and/or independent contractors which APM R deems necessary or desirable for the smooth and efficient
operation and maintenance of the Facilities. Any complaints regarding such personnel shall be directed to APM R's executive director,

general manager or such other person as APM R may designate. No person other than designated employees of APM R shall
reprimand or admonish such personnel or direct them in performing their duties.
