E. CHECKLIST;

DUTIES AND RESPONSIBILITIES YES NO REMARKS


1. Assumed post, duties and responsibilities as Cadet 
Mess Supervisor at about June 1, 2016
2. Accounted all cadets with status and those who are 
excused from formation/messing.
3. Accounted all cadets availed early mess/late mess. 
4. Supervise the platoon formation before leaving the 
Mess Hall.
5. Accounted and recorded the total number of cadets 
entered the Mess Hall in every messing.
6. Reported all cadets without authority entered the 
Mess Hall.
7. Informed the Mess Personnel regarding the number 
of cadets who had their packed meals.
8. Checked, prepared and secured the sound system in 
every messing.
9. Made necessary courtesy when an officer or OD 
enters the Mess Hall.
10. Reported all cadets who are not wearing their proper 
uniforms when entering the Mess Hall.
11. Enforced the proper placements of dining utensils. 
12. Ensured that the areas and seat assignments are 
properly implemented.
13. Checked and activated the ventilation facilities before 
and during mess.
14. Supervised the activities of cadets in the Mess Hall in 
order to follow the Mess Hall regulations.
15. Assisted the Mess Council members in checking the 
daily expenditures, profit and loss statement.
16. Checked the proper allocation of food considering that 
the cadets who were duties availed early mess.
17. Checked the proper sanitation, food preparation, 
maintenance and cleanliness of the Mess Hall.
18. Accounted all Mess Council entering the Mess Hall 
and let them sign in the logbook.
19. Reported all cadets including Mess Personnel who 
caused damage to any Mess Hall utensils/facilities.
20. Reported all problems encountered with regards to 
the food served.
21. Reported to the Mess Personnel the total number of 
cadet’s availed WEP, OB, IL and EL.
22. Prepared a separated table for Honor Code violators. 
23. Submitted CMS After TOD Report to the following: 
a) Cadet Officer of the Day; and 
b) Mess Council Chairman. 
23. Reported all violation of regulations that come to my 
attention by issuing delinquency report.
24. Turned over my post, duties and responsibilities at 
about June 2, 2016 to the incoming duty CMS and
MHS.

Submit this checklist immediately after TOD to the Cadet Officer of the Day.
