III. Background Information

Mary Jane Marcos, a minor, was born at Pasay City General Hospital on November 05,
2014 to MARIPAZ U. MARCOS who is unmarried. She was born through normal delivery
and assisted by DR. PIA CENTENO, Medical Officer III of Pasay General Hospital.

Said minor was admitted to the proper care and custody of the Department of Social Welfare
and Development (DSWD for brevity) by her biological mother. MARY JANE was only 2
days old when she was brought for admission at the center. She was born through normal
delivery and assisted by DR. PIA CENTENO, Medical Officer III of Pasay General Hospital.

The biological mother of the subject minor, MARIPAZ U. MARCOS, was determined that
committing the child to the care and custody of DSWD will be for the best interest of the
child. The biological mother, being a guest relation officer in a KTV Bar has no stable income
to provide the needs of her child. Further, she is not yet prepared to deal with the duties and

obligations of motherhood as she herself can barely provide for her own needs. The
biological mother lives alone and also has no other known family.

As to the father of the child, his identity is unknown.

IV. Medical and Developmental History of the Child

The subject minor was born through normal delivery and assisted by DR. PIA CENTENO,
Medical Officer III of Pasay General Hospital. However, despite of being born healthy, the
child became weak due to lack of proper nutrition.

The child’s biological mother was unable to attend to the needs of the child such as milk,
and other hygiene needs. In fact, the child had skin infection when the mother did not care
to change the child’s undergarments for the whole day despite the need to.

V. Termination of Parental Rights

The biological mother, MARIPAZ U. MARCOS, signed a deed of voluntary commitment


when she admitted the child to the proper care and custody of the Department of Social
Welfare and Development (DSWD) on November 8, 2014.

In such deed, believing it would be for her own interest and benefit, the biological mother
knowingly and willingly relinquished parental authority over the child to the DSWD.

V. Recommendation

The minor was deprived of parental love and care. Since the minor received certification
declaring them legally available for adoption she be placed in a permanent home through
legal adoption at the soonest possible time.

Spouses Florante Sanchez and Elvira dela Cruz Sanchez were found to be acceptable and
were matched by the DSWD. It is for the best interest of said minor if they would be
considered as child of said spouses because her financial, spiritual, and emotional needs
would be taken care of and her future would be safeguarded.
