Pre-Training Questionnaire

OVERVIEW & PURPOSE

The desired outcomes of this training intervention are to foster confident employees who are
prepared and willing to engage with their supervisors and managers honestly, respectfully, and
constructively. In addition, supervisors and managers will be able to respond in an appropriate
manner to absorb, reflect on, and utilize the feedback provided to them by their employees.

PRE-TRAINING QUESTIONNAIRE

1. The facilitator will distribute the Pre-Training Questionnaire to the audience prior to the
training.
2. The facilitator will determine the prior knowledge about upward feedback and the level
of comfort and concerns with delivering upward feedback.

UPWARD FEEDBACK PRE-TRAINING QUESTIONNAIRE

How comfortable are you with providing feedback to your supervisor?

1 – Not at all comfortable. Would not even try to give feedback.


2 – Slightly comfortable. If asked directly, I might give feedback.
3 – Moderately comfortable. I would give feedback to my supervisor if asked
to do so.
4 – Comfortable. I would give feedback to my supervisor.
5 – Extremely comfortable. I have no problem giving feedback anytime.

When engaging in conversation with your supervisor, how often do you provide feedback?

B. 1 - 2 times per quarter
C. 3 - 4 times per quarter
D. 5 or more times per quarter
