AFFIDAVIT OF SUPPORT FROM THE GUARANTOR

I, RICHARD IGLIP ONG, of legal age, Filipino, married, with address at #792 Purok 2,
Brgy. Gogon, Legazpi City, Albay, after having been duly sworn to in accordance with law, do
hereby depose and states:

1. That I am the husband of GENESIS H. PEREZ-ONG as evidenced by a Report of Marriage

issued by Philippine Statistics Authority.

2. That my wife and I will be travelling to Japan on July 27 to August 3, 2018. While in Japan, I
will guarantee the following:

a. Expenses for our stay in Japan;

b. Return Travel Expenses;
c. Compliance with Japanese Laws and regulation.

3. This affidavit is executed to attest to the foregoing statements and in compliance with the
requirements for travel abroad and for whatever legal intents and purposes this may
serve.
