On 01/05/2017, 12:23:36.6 HRS ,Unit # 3 was running at load 315 MW ; Mills-


A,B,C,D,E in service.CW Pump-3A & 3B in service. CW Pump #3/4C kept in
INTERLOCK (for U#3 & U#4). CW-3B Motor Current: 197.67 AMPS. 11 CT fans in
service with risers 100 % Open condition. Cond. Vacuum= -88.27 Kpa. Max.
vibration at Motor NDE X)-1.20mm/s.
Alarm appeared CW Pump #3B system trouble in Annucition and
LVS Pannel @12:23:34.6 Hrs. It was found that CW-3B current dropped to72.42
amps and pump tripped on shaft shear logic (Current <100 amp and disch press <0.5
ksc)@12:23:36.6Hrs. Condencer CW inlet pressure drops from 1.80 to 0.0 ksc. so
ACW Pump B & C tripped on suction press. low @12:23: 40Hrs. CW Pump #3/4C
pickup in interlock for U#3 @12:23:37.9 Hrs.Condensor Vacuum starts dropping.

Immediately Mills 3A and 3E stopped @12:24 Hrs.and generation reduced from 315
MW to177 MW. Vacuum dropped upto -85.54 Kpa and starts improving and hold at
- 89.77 KPa. Mill 3E and Mill 3A Started @12:34hrs and 12:41 Hrs respectively. Load
normalised upto 270MW generation.ACW pump A & C started.@12:57Hrs.

On28/12/2017, TIME 02:52:09 , UNIT # 4 WAS RUNNING @ 218 MW WITH 4


(A,B,C,E) MILLS IN SERVICE. ID FAN 4A AND 4B WAS RUNNING NORMAL IN VFD
MODE WITH FURNACE DRAFT OF -0.1 Kpa. ID FAN B RUNNING @437 RPM, TOTAL
AIR FLOW 768 T/H, TOTAL COAL FLOW 89.22T/H, FAN DCS VIBRATIONS ARE AS
FOLLOWS: MOTOR DE (X) 0.46 mm/s, MOTOR NDE (X) 0.46 mm/s, FAN DE (X) 0.63
mm/s & FAN NDE (X) 0.74 mm/s.
1. AT 02:46:02 MILL 4D STARTED AFTER LUB OIL FLOW SWITCH
WORK FOR MILL CHANGE OVER WITH MILL E.
2. AT 02:52:09 ID FAN B FAN NDE VIBRATION HIGH ALARM CAME IN DCS (ALARM
VALUE : 4.6mm/s, TRIPPING VALUE : 7.1 mm/s).
3. PARAMETER ARE AS FOLLOWS: FAN RPM: 473, TOTAL AIR FLOW : 811T/H, TOTAL
COAL FLOW : 99.5T/H, FAN DCS VIBRATIONS- MOTOR DE (X) 2.05 mm/s, MOTOR
NDE (X) 2.22 mm/s, FAN DE (X) 3.408 mm/s & FAN NDE (X) 5.16 mm/s.
4. ABNORMAL SOUND ALSO OBSERVED AT DE BEARING DURING LOCAL FAN
ROUND.1. FAN RPM REDUCED TO 400 STILL NO CHANGE IN VIBRATION IN DCS.
2. CONDITION MONITORING DONE WITH OSTS.
3. GENERATION REDUCED TO 190MW, MILL A STOPPED 06:35 HRS,FD FAN B
STOPPED @06:42HRS, ID FAN STOPPED @ 07:04 HRS. Fan DE
bearing inner race fully cracked/fractured and erosion of surface (suspected due to
rollers hitting the cracked surface)at crack interphase observed..

On 05/01/2018, 14:57:11.4 HRS , Unit # 4 was running at load 253 MW ; Mills-


B,C,D,E in service.CW Pump-4A & 4B in service. CW Pump #3/4C was Running for
U#3 due to CW Pump 3A Screen cleaning work u/p (PTW ). CW-4B Motor Current:
195 AMPS. All CT fans in service with all risers 100 %. Cond. Vacuum= -91.76 Kpa.
Max. vibration at Motor NDE (x)- 1.19 mm/s and Thurst Brg. temp. - 62*C
Alarm appeared CW Pump #4B system trouble in Annucition and LVS Pannel. It was
found that CW-4B pump tripped on Thrust Brg. Temp. HH logic ( Thrust Brg. Temp.
HH >> 75 *C). Condencer CW inlet pressure drops from 2.18 to 1.29 ksc. so ACW
Pump A tripped on suction prss. low. Condensor Vacuum starts dropping.
Immediately Mills E stopped @
14:57:56.8 hrs and generation reduced from 254 MW to 193MW. Vaccum Pump 4A
started @ 14:58:14 hrs.Vacuum dropped upto -90.86 Kpa Load normalised to 200
MW generation.

On 05.01.2018 16:00 Hrs , Unit# 3 running at 257 MW, and 6.6 kv unit bus U2A
was charged from U1B bus. In U#2, when PA fan 2A started it's breaker got flash
over and same time U#1 tripped as GTCB opened. 6.6kv bus U1B, C1A, C2A, C1B
also got tripped due to which following equipments got tripped.1) Main plant
compressors C,D & E ( Comp. A&B were not avavilable as itssupply bus also got
tripped). 2) CCW pump 3A ,3B Tripped. 3)AHP/CHP , SSS , DMP, FPPH Incommer
Trip. 4) BFP 3C and CW PUMP 3/4 C Tripped. 5) ACW 3A,3B,3C Tripped. 6) PA 3A
Tripped (HC working oil O/L Temp high).7)Sea Air fan 3-B pickup in interlock.
8) BFP 3C
Tripped (HC working oil O/L Temp high). 9)ID 3B Tripped (HC workingoil O/L Temp
high) 10)FD 3B Tripped.
11) Bottom Ash System unit 3. 12) CCW 4C Tripped.
1.Mill 3A stopped and load Reduces up to 150 MW From 265 MW 2.CW PUMP
3/4 C U#3 side interconnection mov manually closed. 3. Station bus C1B charged
from C2B, andC1A bus charged from U3B, than bus C2A charged from C1A (@ 16:12
hrs). 4. CCW pump's 3B, 3C started (@ 16:14 hrs)5. BFP 3A Started (16:19 hrs). 6.ID
3B Started (@ 16:20 hrs) 7. ID 3B Started (@ 16:20 hrs) 8.FD FAB 3B Started @
16:24 9. CW 3A Started @ 17:08 hrs .10. ACW3A,3B. Started @17:22 .
11.AHP/CHP , SSS ,
DMP, FPPH Incommer Charge & Bus coupler made open.
