Dearest Scholar,

The Executive Committee of the U.S. Embassy Club (USEC) Manila is pleased to inform you that
your application has been approved for a volunteer-based scholarship for your college studies.
This scholarship is awarded to you based on your academic rating, volunteer projects, and your
written essay.

Over the next year, you will be busy with your studies as well as your volunteer projects. Please
know that you are a great inspiration to everyone around you.

We look forward to seeing your outstanding achievements over the next year! Remember to
study hard, continue volunteering for those in need, and have fun while doing it!

Academic Year 2018-2019

This AGREEMENT is made and entered into by and between:

U.S. Embassy Club (USEC) Manila, a non-profit organization duly organized and existing under
and by virtue of the laws of the United States of America, with principal office at U.S. Embassy
Manila, Roxas Boulevard, Metro Manila, represented in this instance by the USEC Executive
Committee, hereinafter referred to as the “GRANTOR”;

AND

ERWIN CATABONA, of legal age, parent or guardian of the scholar, and


NOVELL CATABONA, the scholar also referred as the DEPENDENT, both residing at Quezon City,
Philippines, hereinafter referred to as “GRANTEES”.

WITNESSETH:

WHEREAS, the GRANTEE, who is a locally employed staff of U.S. Embassy Manila has applied for
a scholarship grant to finance his/her dependent’s college studies;

WHEREAS, the GRANTOR, upon evaluation of the scholarship application has approved the
scholarship grant, subject to the terms and conditions hereinafter set forth;

WHEREAS, the GRANTOR shall only grant Php 15,000.00 for the scholarship award;

WHEREAS, the aforesaid terms and conditions hereinafter set forth are acceptable to the
GRANTEE;

NOW, THEREFORE, for and in consideration of the foregoing premises and mutual covenants,
the parties hereby agree as follows:

1. The GRANTEE shall submit to the GRANTOR an official or certified true copy of the
DEPENDENT’s school assessment form or verification of payment as basis of payment by
the GRANTOR within 30 days upon receipt of scholarship funding.

2. The DEPENDENT of the GRANTEE shall carry the full academic load for college level, and
of not less than 18 units during a given semester or the equivalent of a full semester load
as prescribed in the school curriculum for college level.

3. The DEPENDENT of the GRANTEE shall be law-abiding and shall not be the subject of any
disciplinary or legal action in or out of school.

4. The DEPENDENT of the GRANTEE shall maintain a general average of at least 85% or its
equivalent, and should not receive an “incomplete,” “conditional,” “failing,” or “dropped”
mark in any subject. Failure to comply with this condition shall result in the scholarship
grant being suspended.

5. Submission of falsified or tampered documents by the GRANTEE or his or her DEPENDENT


shall automatically and permanently disqualify the GRANTEE and DEPENDENT from the
program including future considerations.

6. If the GRANTEE resigns or is terminated by the U.S. Embassy Manila where he is employed
for any reason, the scholarship grant to his/her DEPENDENT shall automatically be
cancelled.

7. The GRANTOR reserves the right to terminate the scholarship grant at any time if the
GRANTEE is held liable for grave misconduct in the U.S. Embassy Manila where he is
employed; or in case of violation of any of the terms and conditions of this AGREEMENT.
The termination contemplated herein shall be without prejudice to the right of the
GRANTOR for reimbursement of the full amount paid for by the GRANTOR under the
scholarship grants availed by the GRANTEE’s DEPENDENT.

8. The GRANTOR shall have the right to terminate or withdraw the scholarship grant to the
GRANTEE’s DEPENDENT at any time for lack of funds.

9. The GRANTEE agrees that his or her DEPENDENT will complete 20 hours of volunteer work
with the local community, charity or non-school sponsored volunteer activities per
semester with a total of 40 hours of volunteer work for the school year. This is to include
no less than 4 hours and no more than 8 hours of volunteering with the U.S. Embassy
Club. (Community service volunteer work hours will be recorded in a time sheet that will
be submitted to the USEC Program Manager for cross-checking including photos as proof
of volunteer participation.)

10. The GRANTOR shall only disburse the scholarship funds in two (2) 50% disbursements;
the initial 50% disbursement will be disbursed upon awarding. The second 50%
disbursement will be contingent upon the receipt of a student transcript with an 85% or
higher grade during the first semester, and the completion of the required community
service hours – 20 hours of volunteer work hours for each semester.

11. The GRANTOR shall have the right to disqualify the GRANTEE’s application for the next
USEC Community Service Scholarship if the GRANTEE fails to provide the proof of
completion of 20 hours of volunteer work hours during the second term.

12. The GRANTOR shall have the right to require submission of a testimonial about his or her
experiences as a Community Service Scholar from the DEPENDENT. Whether it be in the
form of a picture, video, write-up, or any other format the GRANTOR indicates.

13. The GRANTOR reserves the right to post pictures or videos of the DEPENDENT for usage
in U.S. Embassy Club’s official website, social media accounts, and other mediums for
posts related to the Community Service Scholarship.

14. The GRANTEE agrees that his or her DEPENDENT shall be present during U.S. Embassy
Club sponsored events for recognition.
